Objective: It is to analyze the relationship between blood pressure variability and carotid intima media thickness(cIMT),ankle brachial index(ABI),carotid-femoral pulse wave velocity(cfPWV)in Patients with Essential hypertention.Methods: Select patients with essential hypertension in 105 cases,which include 54 males and 51 females,aged from 42 to 83 years old,with the average age(65.2±8.09)years old.Application of carotid duplex ultrasound to monitor the carotid intima-media thickness(cIMT),the ankle brachial index(ABI)and carotid-femoral pulse wave velocity(cf PWV)were measured by limb doppler perpheral vascular tester.According to the test results,the patients were divided into cIMT normal group,cIMT thickening group,ABI normal group,ABI abnormal group,cfPWV normal group,cf PWV abnormal group.The patients were underwent examination of the 24 h ambulatory blood pressure monitoring.The results of the blood pressure level and blood pressure variability were compared between the two groups,At the same time the patients’ age,sex,body mass index(BMI),smoking history,hypertension duration,triglyceride(TG),high-density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C)were also recorded.Results: 1.The levels of 24 hSSD,d SSD,n SSD,24 hSBP,24hDBP,d SBP,d DBP and n DBP in cIMT thickening group were significantly higher than thoose in cIMT normal group(P<0.01);The levels of d DSD and n SBP in cIMT thickening group were significantly higher than those in cIMT normal group(P<0.05).There was a positive correlation relationship between 24 hSSD,d SSD,d DSD,n SSD,24 hSBP,24hDBP,d SBP,n SBP,n DBP,n DSD,d DBP and cIMT.Line to cIMT level as the dependent variable,multiple stepwise regression analysis showed that d SSD,24 hSBP,d DBP,d DSD and n SSD entered regression equation.2.The levels of 24 hSSD,d SSD,n DSD,24 hSBP,d SBP and n SBP in ABI abnormal group were significantly higher than those in ABI normal group(P<0.01);The level of d DSD in ABI abnormal group was significantly higher than that in ABI normal group(P<0.05).There was a negative correlation relationship between 24 h SSD,d SSD,d DSD,n SSD,n DSD,24 hSBP,d SBP,n SBP,n DBP and ABI.Line to ABI level as the dependent variable,multiple stepwise regression analysis showed that 24 hSBP and d SSD entered regression equation.3.The levels of 24 hSSD,d SSD,24 hDBP,d DBP,n DBP in cfPWV abnormal group were significantly higher than those in cfPWV normal group(P<0.01);The level of n SSD in cfPWV abnormal group was significantly higher than that in cfPWV normal group(P<0.05).There was a positive correlation relationship between 24 hSSD,d SSD,d DBP,n DBP,BMI,24 hDBP and cfPWV.Line to cfPWV level as the dependent variable,multiple stepwise regression analysis showed that 24 hSSD and d DBP entered regression equation.Conclusion: The blood pressure level and blood pressure variability are closely related to cIMT,ABI and cf PWV.The increase of blood pressure variability is associated with accelerated cIMT,ABI and cfPWV becoming abnormal.Blood pressure variability in a certain extent can be used as an important index to predict the cardiovascular events and target organ damage.
