Objective To explore the influence of NICU related system and facilities in China,we take a semi open visiting system and carry out family centered nursing plan to observe its influence on parents' care ability of premature infants.Methods From January 2016 to December 2017,112 cases of premature infants admitted to the neonatal intensive care unit(NICU)met the study criteria.After their informed consent,they were randomly divided into two groups.The control group 53 case sreceived routine admission,admission nursing and discharge guidance;while the 59 cases of observation group were provided extra family centered nursing(FCC)scheme on the basis of routine nursing.FCC scheme is defined as training of the related nursing knowledge and skills to guide the parents of premature infants,and arrange for premature infants' parents stay home nursing ward in the hospital during infants' parents weekly visitation time by the duty nurse guidance and demonstration,learning and take the main daily care of preterm infants.Results? In the control group,there were 53 preterm infants,with 30-36 weeks of fetal age,1350-2900 g in body mass,38-48 cm in length,and 14-45(22.454 ± 5.729)d in hospital time.The observation group of 59 premature infants were with gestational age of 29-35 weeks,1280-2920 g in body mass,35-50 cm in length,and 15-48(23.273 ± 6.245)d in hospital time,including family ward hospitalization time of 1-4(2.321 ± 0.832)d.There was no statistical significance(P>0.05)between the two groups in premature sex,gestational age,mode of delivery,parents' age and culture degree,? The average birth weight of the two groups were 1895.26 ± 321.43 g and 1864.32 ± 475.52 g,respectively.The average length was 43.54 ± 2.67 cm and 42.82 ± 3.91 cm,respectively.The discharged average weight was 2336.32 ± 235.35 g and 2403.61 ± 368.36 g,respectively.The average length was 45.46 ± 2.53 cm and 45.63 ± 2.95 cm,respectively.During subsequent visit,the average weights of the two groups were 3048.84 ± 401.78 g and 3126.58 ± 425.73 g,respectively.The average length were 49.74 ± 2.76 cm and 50.36 ± 2.84 cm,respectively.These data showed no significant differences(P> 0.05).? The control group and the observation group premature admission self care knowledge scores were 21.53 ± 3.36 and 21.78 ± 3.91;self care scores were 18.64 ± 4.27 and 18.31 ± 4.18;self care ability score were 10.65 ± 2.93 and 10.42 ± 2.89 points,the difference between the two groups had no statistical significance(P>0.05);the control group and the observation group of premature infants at discharge self care knowledge scores were 22.32 ± 4.68 and 24.14 ± 4.91;self care scores were 20.31 ± 5.36 and 24.65 ± 5.83;self care ability score were 12.26 ± 3.17 and 18.18 ± 3.23,the difference between the two groups was statistically significant(P<0.05).? The most problems reflected by parents were milk regurgitation and cough.37 cases occurred in the control group,while 28 cases in the observation group,with ratios of 71.70% and 47.46%,respectively.Each cases had 1-5 times milk regurgitation or cough,showing significant difference between the two groups(P<0.05).How to avoid milk and cough,and the subsequent processing of these problems were the most important consideration of the parents.The incidence of diaper rash in the control group and the observation group were 20.75% and 3.39%,respectively,showing statistically significant difference(P<0.01).Because of the change of the original condition,2 cases of the control group were hospitalized after discharge within 1 week,with a proportion of 3.77% of the control group.The rest of the premature infants were not hospitalized for 1 months because of improper nursing.Conclusions ? The family centered nursing model can improve the parental care ability of preterm infants.? The family centered health education model can reduce the incidence of abnormal conditions in preterm infants after discharge.? During hospitalization,parents are allowed to participate in the nursing work of premature infants,so that parents can master more nursing knowledge and skills,improve the care technology of premature infants,and have the ability to become the primary caregivers of premature infants.? The short-term effect of the family centered nursing model on the growth and development of preterm infants is not obvious.
