Objective:Overweight and obesity lead to serious health consequences,and it is especially closely related to cardiovascular events,which is one of the high risk factors for heart disease.At present,the effect of obesity on patients with unstable angina is unclear,and the effect of obesity on plasma NT-proBNP values in patients with unstable angina(UA)is also not clear.The major purpose of this study is to investigate the correlation between the concentration of plasma NT-proBNP and the body mass index(BMI)in patients with unstable angina pectoris.Methods:1?A total of 560 patients with unstable angina who were hospitalized in the cardiology department of Affiliated hospital of yanbian university in Jilin province from January 2014 to December 2017 were selected(238 were overweight or obesity and 322 were normal weight),age from 31 to 94,mean age(66.61±9.84).Among them,326 greater than or equal to 65 years old and 234 under 65 years old;281 male and 279 female.All subjects were excluded from infection,valvular heart disease,chronic obstructive pulmonary disease,severe hepatic and renal insufficiency,cardiomyopathy and malignant tumor.All of the patient's The general condition(such as age,gender,BMI),the history of other diseases and bad habits(history of hypertension,diabetes,smoking),chemical test results after admission(such as lipid examination,creatinine,blood pressure,heart rate,NT-proBNP)and other data were recorded for the research.In this study,the patients with BMI ?25 were classified as overweight or obese patients and the others were classified as normal weight patients.2?The plasma NT-proBNP concentration was determined by colloidal gold method.SPSS Statistics medical statistics software was used for the statistics.The averages of All datas were expressed by(Mean±standard error).We used the logarithm of NT-proBNP(Log NT-proBNP)for instead,as the NT-proBNP numerical was relatively large.Pearson X2 test was used for Constitution ratio of two samples.About the comparison between groups,we selected the analytical methods based on the sampling distribution:Independent sample t-test was adopted for normal distribution,Wilcoxon rank sum test was adopted for skewed distribution,and Linear regression analysis equation was used for the further determination of the linear relationship between BMI and Log NT-proBNP.Results:1.The average age of the group of unstable angina pectoris(UA)with overweight or obese(BMI?25)patients was(64.08?9.13)years,the average age of the group of unstable angina pectoris with normal weight(BMI<25)patients was(68.48±9.95)years,and the P value was statistically significant(P<0.001).2.The average of plasma NT-proBNP concentration of UA patients with BM?25 was(231.86±215.06)pg/ml,average of Log NT-proBNP was(2.18±0.42);The average of plasma NT-proBNP concentration of UA patients with BMI<25 was(370.08±367,49)pg/ml,average of Log NT-proBNP was(2.36±0.44).The P value was statistically significant(P<0.001),comparing datas of the two groups.3.The results of the rank sum test:The median of group of UA patients with BMI?25 was 158.36(70.37,326.25);The median of group of UA patients with BMI<25 was 233.93(100.00,523.25),The P value was statistically significant(P<0.001),comparing datas of the two groups.4.The results showed that the effects of age and creatinine were the most obvious,and the p values were less than 0.001.Followed by hypertension and ethnic factors,the P values were less than 0.05,which was statistically significant.5.In addition to creatinine abnormality,the concentration of NT-proBNP in the obese group was significantly lower than that in the normal group,and the p values were less than 0.05,which was statistically significant.5.According to the linear regression result:Body mass index is negatively related to the concentration of plasma NT-proBNP in patients with unstable angina(r value-0.182,P<0.001),there is obvious linear relationship between body mass index(BMI)and the concentration of plasma NT-proBNP in patients with unstable angina.Conclusion:1?The plasma NT-proBNP concentration of UA patients with BMI ?25 was significantly lower than UA patients with BMI<25.Body mass index(BMI)was negatively correlated with plasma NT-proBNP concentration,and under the same condition,the more obese,with higher BMI,the lower plasma NT-proBNP concentration showed.2?Age,ethnicity,hypertension and creatinine may affect the plasma NT-proBNP concentration of patients with unstable angina(UA),among those factors,age and creatinine abnormality were most strongly related to high NT-proBNP levels,while,there was no significant relationship between plasma NT-proBNP concentration and the factors of gender,smoking history,diabetes,blood lipids.3?Overweight or obesity make the onset age of unstable angina more younger.
