Background and SignificanceHenoch-Schonlein purpura is one of the most common childhood autoimmune vasculitis disease.The clinical features were non thrombocytopenic purpura,arthritis,gastrointestinal symptoms,Among them,the important factors affecting prognosis are renal involvement and proteinuria and/or hematuria,namely Henoch-Schonlein purpura nephritis(HSPN).To date,the mechanism of HSP and its renal damage has not been clearly defined.Wnt/beta-catenin signaling pathway is a more complex signaling pathway in biological evolution,and is involved in many physiological processes of life,and plays an important role in tumor development,organ fibrosis and other pathological processes.Research shows that in rheumatic diseases exist in the activation of Wnt/beta-catenin signaling pathway,plays an important role in the occurrence and development of the signaling pathway in acute renal tubular injury of renal tubule regeneration and repair,renal tumors,and found that the formation of the Wnt signal transduction pathway and renal fibrosis is closely related Henoch-Schonlein purpura is the most common rheumatic disease in children at the same time,prognosis and kidney damage is closely related to the existence of the allergic purpura the activation of Wnt pathway,this study was examined by immunohistochemistry Wnt/beta-catenin signaling pathway in the skin and kidney tissue HSP,A preliminary study of whether the Wnt signaling pathway is involved in the development of HSP and its renal damage.Materials and methods Research object:select Medical University Of Anhui HSP children from January 2014 to January 2016 were the First Affiliated Hospital of 9 cases and 8 HSPN cases,the skin tissue in HSP patients and their families obtain informed consent,male 5 cases,female 4 cases,aged 3.5-13 years old(8.5),normal control group obtained by the HSP group and the Department of burns;the normal control group age and gender difference was not statistically significant.At the same time,we collected kidney tissues from patients with renal involvement,including 4 males and 4 females,whose age was 9-13(11.131.81)years old.The normal control kidneys were obtained from our hospital pathology department.There was no significant difference in age and gender between HSPN group and normal control group.The skin and renal tissue were treated with paraffin embedding.At the same time,the urine protein was calculated for 24 hours in children with HSPN.According to immunohistochemical kit instructions to detect the expression of HSP in kidney tissue of skin tissue,HSPN Wnt4,beta-catenin and E-cadherin,the positive reaction known as positive control,negative control for PBS instead of a resistance,each section were observed 5 times 400 lens vision.At the same time,the results of immunohistochemistry were measured by the Image-Pro Plus image analysis software system of the Media Cybernetics company in the United States.The integral optical density(IOD)values of Wnt4,beta-catenin and E-cadherin in every field were measured by Image-Pro Plus image analysis software,and the average values of Wnt4,beta-catenin and IOD were calculated.Statistical analysis:data analysis using SPSS 17.0 statistical software,the normal distribution test data,the results obtained were in normal distribution,(x±s)said that among the groups of sex compared by chi square test,correlation analysis using Pearson correlation analysis,data analysis and comparison between the two groups by t test.P<0.05 had statistically significant difference.ResultThe expression of Wnt4,beta-catenin and E-cadherin in group HSP group skin:the expression of Wnt4 and beta-catenin in group HSP was mainly located in the cytoplasm and cell membrane,and low expression in the normal control group,mainly located in the cell membrane,the expression of Wnt4 and beta-catenin in the HSP group of skin is higher than the normal control group(P<0.01);the expression of E-cadherin in group HSP,mainly located in the cytoplasm,the expression of E-cadherin in the normal control group,mainly located in the cell membrane,the expression of E-cadherin in the HSP group of skin lower than the normal control group(P<0.01);The expression of Wnt4,beta-catenin and E-cadherin in renal tissues of HSPN group:the expression of Wnt4 and beta-catenin in group HSPN was mainly located in the cytoplasm,mainly expressed in renal tubules,and less expression in the normal group,the expression of Wnt4 in kidney tissue in HSPN group is higher than the normal control group(P<0.01),beta-catenin expression of kidney tissue in the HSPN group was higher than that of normal control group(P<0.01);the expression of HSPN in E-cadherin is mainly expressed in renal tubule,cell membrane and cell gap and normal kidney tissue of E-cadherin was mainly expressed in renal tubular epithelial cells,the expression of E-cadherin in kidney tissue in HSPN group is lower than the normal control group(P<0.01);see Fig.1 and table 2;there was a negative correlation between the amount of 24 hours urine protein E-cadherin expression in the HSPN group(r=0.695,P<0.05).Conclusion Wnt4,beta-catenin and E-cadherin have abnormal expression in HSP skin and kidney tissue.Meanwhile,the expression of E-cadherin in renal tissue is negatively correlated with urinary protein.It is speculated that Wnt/beta-catenin signaling pathway may be involved in the pathogenesis of HSP and renal involvement.
