Clinical Research On The Biopsy Of Intrantable Cases In Sellar Region

Background:Sellar region lesion is a common disease in neurosurgery.It has been hot topic of neurosurgery study because of its complicated structures,rich blood supply and variety diseases.With the intensive study of the saddle area disease,most common diseases in this area are easily diagnosed in the clinic such as pituitary adenoma and craniopharyngioma.But another diseases with low incidence and lack of clinical characteristics have become the difficulties in the diagnosis and treatment of sellar region diseases.A retrospective analysis of diagnosis and treatment of intrantable cases at QiLu hospital ShanDong University between December 2014 and October 2017 was conducted.We found that sellar region lesion biopsy is a microinvasion and highly beneficial operation for the patients.After obtained the pathological results with biopsy,most patients can be treated by conservative treatment or other non-operative therapy,which have achieved better prognosis.This surgery is worthy of clinical use and spread.Objective:To research the clinical characteristics of intrantable cases,and to discuss the value of the biopsy in the diagnosis,treatment and prognosis of difficult diseases.Methods:We performed a retrospective analysis of diagnosis and treatment of intrantable cases at QiLu hospital ShanDong University between December 2014 and October 2017.The features of age,symtom,imaging and endocrine were summarized,combined with the pathological results which including 4 cases of germ cell tumors,3 cases of metastatic tumors,4 cases of pituitary inflammation,1 case of non-Hodgkin B-cell lymphoma,1 case of optic glioma,1 case of small cell pituitary cell tumor,1 case of hairy cell astrocytoma,1 case of low malignant pituitary source tumor,1 case of pituitary adenoma complicated with craniopharyngioma,1 case of Chordoma 1 case of pituitary adenoma,1 case of craniopharyngioma,to discuss the advantages of the biopsy.Results:The pathological results of 20 cases who underwent the sellar region lesion biopsy showed the diversity and complexity of sellar diseases.However,many of them do not require secondary operation,better prognosis have been achieved by radiotherapy,chemotherapy or intravenous drug therapy.The biopsy has the advantages of minimally invasive,slight postoperative clinical symptoms,short hospital stay and fast recovery.It can entry the next therapy stage early and is worthy of clinical use and spread.Conclusions:1.Sellar disease have many kinds and untypical clinical symptoms so that it is difficult to diagnose preoperatively.2.The biopsy can be used as a therapeutic method for the intrantable cases.The clinical pathological results are obtained by minimally invasive surgery to guide the further stage of treatment.3.The biopsy has the advantages of short operation time,small trauma,fast recovery and less cost,it retained the chance of extended resection,which is worthy of clinical promotion.
