Objective:To collect clinical data of 24-hour urine metabolic test of upper urinary tract calculi in our center and analyze its metabolic disorders;to evaluate the safety and effectiveness of potassium citrate sustained-release tablets to treatment 24-hour urine metabolic disorders in patients with upper urinary tract calculi.Methods:This study selected patients who underwent urinary calculi surgery no stone residues were found and the maximum diameter of the residual stone were less than4 mm after surgery from urology of the Second Affiliated Hospital of Zhengzhou University from August 2016 to March 2017.100 patients who met the inclusion and exclusion criteria were selected,and collected urine for 24 h to detect the content of various components in their urine.Random digital table were used to randomly group the qualified patients(treatment group and blank control group).treatment group:control group = 1:1,for example,50 patients(including 10% shedding rate)were gave oral potassium citrate sustained-release tablets in the treatment group,1.08g/tablet three times a day;blank control group took test drugs;requirement of both groups daily urine volume and water intake during the study be greater than 2000 ml,and according to the results of their stone composition analysis of the two groups patients to adjust the diet and health education,the observation period is 1 year,a total of 5 observation points.Results:100 patients were included in the study,of which 97 patients successfully completed the study,3 patients fell off,and the removal rate of shedding was 3%.There was no significant difference in gender,age,stone size,stone location,stone side,and stone composition between the treatment group and the control group.100 patients with upper urinary tract calculi,with 97 had metabolic abnormalities of urine,52 had low uric aciduria(52%),18 had low urine(18%),14 had hyperuricemia(14 %),22 cases of hypercalciuria(22%),22 cases of hyperoxaluria(22%),and 3cases without significant urine metabolic abnormalities.Of the 100 patients with upper urinary tract calculi,with 68 had abnormal urine metabolism,27 had abnormal urine metabolism,and 2 patients had 3 metabolic disorders;after oral potassium citrate drug 24-hour urine metabolic disorders were improved in the treatment group,and the urine metabolic disorders in the control group were not significantly improved.In addition,it was demonstrated that potassium citrate can reduce the formation of ureteral stent wall-attached stones in patients with upper urinary tract calculi;the drug is safe and effective,no obvious liver and kidney and other organ damage during the study,and suitable for promotion and application.Conclusions:1.Most patients with upper urinary tract calculi in our center have one or two urinary metabolic disorders,mostly low uric acid urine,high calcium urine,high oxalic acid urine,followed by low urine output and high uric acid.2.Potassium citrate sustained-release tablets can effectively treatment the urinary disorders in patients with upper urinary tract calculi.This method is safe and effective and suitable for popularization and application.3.Oral potassium citrate sustained-release tablets can prevent ureteral stent wall-attached stones.potassium citrate sustained-release tablets is suitable for clinical application just with less adverse reactions.
