| Objective: to study the intervention effect of wuwei qingdan combined with western medicine on Gastric ulcer(Gastric ulcer,GU).To provide evidence-based medicine for the treatment of Hp related gastric ulcer and the prevention of recurrent ulcer recurrence.Methods: 80 patients with gastric ulcer who met the admission criteria of this clinical trial were selected and divided into treatment group(40 cases)and control group(40 cases)by random number table method.The control group was given a four-way therapy for the treatment of helicobacter pylori(Hp)for 14 days,and the sequence was given to rabeprazole enteric-coated tablets for 6 weeks.The treatment group was combined with the treatment of "wuwei qingdaofang" in combination with Chinese herbal medicine on the basis of the control group,both of which were 8 weeks.After the course of treatment,the electronic gastroscopy and histopathological examination were completed,and the 14 C breath test was reviewed after 4 weeks of treatment.Compare two groups of symptoms overall curative effect,clinical symptoms integral,the eradication of helicobacter pylori(Hp),endoscopic ulcer healing,ulcer pathological histology regenerative mucosal maturity and safety indicators,such as performance of endoscopic gastric clinical cure+and Hp eradication in patients with long-term follow-up had marked effect,12 months after drug withdrawal to check again respectively electronic gastroscope and 14 c breath test,such as digestive system appears during follow-up of patients with clinical symptoms,the consideration is gastriculcer person also perfect the electronic gastroscope and 14 c breath test,for the long-term curative effect evaluation.Results:(1)clinical efficacy: the total effective rate of the treatment group and the control group was 97.2%,89.5%,and the clinical cure+efficiency was86.1% and 63.2% respectively,and the efficacy of the treatment group was better than that of the control group(P < 0.05).(2)clinical symptoms integral:total points: the treatment group and control group after treatment of clinical symptom score difference was statistically significant(P<0.05),treatment group curative effect is better than that of control group,two groups overall clinical symptom score before and after treatment compared with significant difference(P < 0.01),obvious better than before treatment after treatment.Among various clinical symptoms integral group comparison: the treatment group after treatment the diagnosis hot mouth pain,dry mouth,bitter,body weight,drowsiness,nausea,and vomiting symptoms,less food,stay compared with control group the difference was statistically significant(P <0.05),the treatment group is better than the control group;There were significant differences in the clinical symptoms before and after treatment in the treatment group(P < 0.01),and the treatment was significantly better than before treatment.In the control group,there was a significant difference(P <0.01)before treatment,and the clinical symptom score was significantly higher than that before treatment.There was a statistically significant difference between the symptoms of heavy sleepiness and the symptoms of nachaxia(P < 0.05).(3)after the treatment,the electron gastroscope was reviewed,and the two groups showed significant efficacy in endoscopy of gastric ulcer.The total effective and clinical efficacy of the two groups was not statistically significant(P > 0.05).(4)after 4 weeks of treatment of the two groups,the 14 C exhalation test showed that both groups had better eradication of Hp,and there was no significant difference in the Hperadication rate between the two groups,with no statistical significance(P >0.05).(5)after treatment,the treatment group and the control group had a comparison of the histological maturity of the regenerated mucosa in the pathological tissue,and the rate of "excellent" in the treatment group was higher than that in the control group,and the difference was statistically significant(P < 0.05).(6)discontinuation in December after two groups of gastroscope curative effect of patients with clinical cure+forward ulcer recurrence rate comparison,the treatment group ulcer recurrence rate is lower than the control group,the difference was statistically significant(P < 0.05).(7)in the two groups of Hp eradication patients after 12 months of withdrawal,the recurrence rate of the patients in the long-term Hp was compared,and the recurrence rate of the treatment group was lower than that of the control group,and the difference was statistically significant(P < 0.05).(8)both groups did not have adverse reactions during the treatment,and the blood routine,urinary routine,stool routine,liver and kidney function and ecg were all normal before and after treatment.Conclusion: 1.Combined with western medicine sequential therapy can effectively improve the clinical symptoms of Hp associated gastric ulcer.2.Combined with western medicine sequential therapy can effectively promote ulcer healing,improve inflammatory response,and improve the histological maturity of ulcer regeneration mucosa.3.five tastes bile party combined sequential therapy and pure western medicine western medicine sequential therapy for Hp correlation ulcer comparison,recent either endoscopic ulcer healing and Hp clearance were no obvious difference,but the long-term curative effect of contrast,five tastes pure bile party combined western medicine sequential therapy can obviously reduce ulcer recurrence rate and Hp recurrence rate,is worthy of popularization and application. |
