ObjectiveThe changes and correlation of epoxyeicosatrienoic acids(EETs)were compared between normal and T2 DM and DN patients.To explore whether EETs can be used as an early prognostic indicator of diabetic nephropathy and the entry point of interventional therapy,to provide new ideas for the prevention and treatment of diabetic nephropathy.MethodsAccording to the WHO diagnostic criteria of T2 DM,118 patients with T2 DM who were hospitalized in the endocrinology department of Qingdao municipal hospital from December 2016 and June 2017 were selected.A total of 58 patients with diabetic nephropathy were divided into diabetic nephropathy group(DN group),and 60 cases were non-diabetic nephropathy group(Non-DN group).At the same time,39 healthy people who were examined in the physical examination center of Qingdao municipal hospital were selected as the normal control group(NC group).General clinical data of all subjects were collected,which included age,gender,course of disease,body mass index(BMI);Determination of Fasting plasma glucose(FPG),glycosylated hemoglobin(HbA1c),trigly ceride(TG),total cholesterol(TC),low density lipoprotein(LDL-c),high density lipoprotein(HDL-c),urea nitrogen(BUN),creatinine(Cr),fasting Cpeptide(FCP),urinary micro albumin(UMA),urinary micro albumin/creatinine ratio(UMA/Cre);The lelve of EETs were determined by ELISA.All data were statistically described and inferred using SPSS22.0 software.Quantitative data were analyzed by independent sample t test and variance analysis.Spearman linear correlation analysis was used to determine the relationship betweenclinical indicators,and the influence factor adopts multivariate regression analysis.The test level was 0.05,and P < 0.05 was considered statistically significant.Results1.The results of ELISA showed that: Compared with the NC group,the serum EETs level of Non-DN group and DN group decreased significantly,and the difference was statistically significant(P<0.01).Compared with the Non-DN group,the serum EETs of the DN group decreased significantly,and the difference was statistically significant(P<0.01).2.The serum BUN and Cr levels in the NC group,Non-DN group and DN group showed an upward trend,and the difference was statistically significant(P< 0.05).The serum FPG,HbA1 c and TG levels in the Non-DN group and DN group were significantly higher than that in the NC group,and the HDL-c significantly decreased,the difference was statistically significant(P < 0.01).3.spearman correlation analysis showed that EETs was positively correlated with FCP(r=0.191)(P<0.05).It was negatively correlated with the course of disease(r=-0.692),HbA1c(r=-0.175),TC(r=-0.167),LDL-c(r=-0.163),UMA/Cre(r=-0.292)(P<0.05).There was no significant correlation with age,BMI,FPG,TG and HDL-c(P>0.05).4.With the level of EETs as dependent variable,age,course of the disease,BMI,FPG,HbA1 c,TC,TG,HDL-c,LDL-c,FCP,UMA/Cre as independent variables in multivariate regression analysis,the results show that: LDL-c,FCP were independent influence factor of EETs,and the determination coefficient is 0.039 and 0.074 respectively.Conclusion1.The level of serum EETs in patients with T2 DM was significantly lower than that of healthy people.while those with diabetic nephropathy had lower levels of serum EETs compared with those without DN.Therefore,We conclude that the EETs level in serum is closely related to the occurrence and development of T2 DM and DN.2.Serum EETs level was negatively correlated with the course of disease,HbA1 c,TC,LDL-c,UMA/Cre,and was positively correlated with FCP,LDL-c and FCP are independent influences of EETs.Therefore,EETs level is closely related to DN severity.EETs is involved in the function of islet cells and the regulation of blood glucose and lipid.
