Objective:To search the relationship between the diameter of optic nerve sheath in CT and intracranial pressure,and to provide a new method for clinical medicine without invasive intracranial pressure measurement.Method:This research for the study of the double blind trials,the object of study for our hospital Affiliated Hospital of Hebei University of Engineering in October 2015 to October 2015 suspected intracranial disease checking head CT examination in order to make clear the patients of intracranial lesions,and when skull CT examination of20 min after making lumbar puncture,to measure intracranial cerebrospinal fluid pressure.Each study participant was required to collect individual information such as age,sex,breathing rate,pulse frequency,systolic blood pressure,diastolic blood pressure,and disease name.The subjects were divided into group A and group B according to the classification of craniocerebral injury.Group A included patients with mild and moderate craniocerebral injury.Group B patients with severe and deadly severe craniocerebral injury.And the results of intracranial pressure measured by the subjects were grouped by normal cranial pressure and high cranial pressure,and there was significant difference between the two groups.The correlation between intracranial pressure and the diameter of optic nerve sheath was verified by statistical methods.The optimum diagnostic value of nerve sheath diameter was determined by using the receiver operating characteristic curve.Results:1.The intracranial pressure of patients with severe craniocerebral injury was generally increased,and the study of intracranial pressure was predictive of craniocerebral injury.There was significant difference?P=0.001<0.05?between the optic nerve sheath diameter and the low cranial pressure group at 3mm after the eyeball.2.There was simple linear regression between the optic nerve sheath diameter and the low cranial pressure group at 3mm after the eyeball.The line regression model is D3=0.117*ICP+3.65?D3 for optic nerve sheath diameter CT measurements?.When ICP equal to 20cmH2O,the optic nerve sheath diameter is5.9mm..3.The cut-off of optic nerve sheath diameter CT in the diagnosis of elevated inrtracranial pressure with CT is 6.8mm,as a sensitivity of 59.1%and a specificity of 87.5%.Comparing the cut-off of optic nerve sheath diameter CT in the diagnosis of elevated intracranial pressure with CT is 5.9mm,as a sensitivity of86.4%and a specificity of 56.2%.Conclusion:AS the computed tomography?CT?in our state is commonplace,the optic nerve sheath diameter after 3 mm of eysball in head CT predicts whether brain injury especially severe craniocerebral injury patients with intracranial pressure increased easily,Conveniently,relatively accurately.
