Object:To investigate the association between leukoaraiosis and intracranial large atherosclerotic stenoses or occlusions in patients with ischemic cerebrovascular diseases(including cerebral infarction and transient ischemic attack)and to analyze the risk factors of leukoaraiosis.Methods:176 patients with ischemic cerebrovascular disease(including cerebral infarction and transient ischemic attack)who were admitted to Beijing Tiantan Hospital from October 18 th 2007 to August 30 th 2008 were collected.Patients were enrolled in the study if they had an onset of symptoms within 7 days and were aged between 18 and 80 years.Collect the basic information of the patients(age,sex and so on),previous history(hypertension,hypertriglyceridemia and so on),cranial magnetic resonance imaging and angiography.According to the severity of stenosis,we classified the patients into five groups using the published method for the WASID:normal,<50% stenosis,50%–69% stenosis,70%–99% stenosis,and occlusion.We define the stenosis rate is greater than or equal to 50%,which we include in our study.According to the ARWMC score of leukoaraiosis,the patients were divided into significant leukoaraiosis group(ARWMC>2)and no or mild leukoaraiosis group(ARWMC?2).The Logistic regression was used to analyze the relationship between age,hypertension,total cholesterol,current smoking,history of ischemic stroke,multiple intracranial large atherosclerotic stenoses or occlusions and leukoaraiosis.We define P<0.05 as significant statistical meaning.Results A total of 176 cases were enrolled in the study,average age was 60.43±11.62 years old,including 130 cases of man and 46 cases of woman.There were 94 cases with no or mild leukoaraiosis and 82 cases with significant leukoaraiosis.The data between the two groups indicated that age,total cholesterol,current smoking,history of ischemic stroke and multiple intracranial large atherosclerotic stenoses or occlusions were statistically significant(p<0.05),but the significant statistical meaning was not seen in the sex?hypertension?hypertriglyceridemia or other data(p>0.05).The Logistic regression of multiple factors which included single factors with P < 0.15,indicated that:age(OR=1.099,95%CI 1.060-1.139),multiple intracranial large atherosclerotic stenoses or occlusions(OR=2.441,95%CI1.181-5.046),and total cholesterol(OR=0.685,95%CI 0.511-0.917)were associated with leukoaraiosis.Conclusions The independent risk factors of leukoaraiosis are age and multiple intracranial large atherosclerotic stenoses or occlusions,and the probable protective factor is total cholesterol.There is a significant association between the severity of leukoaraiosis and intracranial multiple atherosclerotic stenoses or occlusions.
