Investigation on the status of parental compliance of neonatal jaundice and analysis of its influencing factors Objective To investigate the status of the compliance of the parents of neonatal jaundice in our hospital and analyze the influencing factors.Methods The parents of the newborns who have jaundice symptoms during the treatment at Outpatient department of Pediatrics of the First Affiliated Hospital of Chengdu Medical College in Sept 1st,2015~Aug 31 st,2016 were investigated.Use the questionnaire method to understand the parents' implementation of the doctor's orders.The compliance assessment was used to evaluate the compliance.(divided into good compliance,Commonly Compliance And poor compliance).The Logisitc multiple factor multiple regression method was used to analyze the related factors of parental compliance.Including mother's age,nation,Place of residence,family per capita monthly income,parents' educational level,account category,mother's pregnancy disease,gestational times,production times,mode of delivery,time to arrive at the hospital,waiting time,waiting for the time to check the result,whether parents are anxious,parents' knowledge of neonatal jaundice knowledge,meconium-stained amniotic fluid or not,presence of premature rupture of membranes,is there fetal heart,placenta,the umbilical cord abnormal,birth asphyxia or not,age in days,the degree of jaundice,complications,the elder brother or sister(if there is)the jaundice in the neonatal period,etc.Results among the 1032 cases of parents surveyed,444 cases were good compliance,accounting for 43%,264 cases of compliance,25.6% and 324 compliance,accounting for 31.4%;Household income,parental educational level,the time it takes to come to our hospital,the waiting time,the time it takes to wait for examination results,maternal complications during pregnancy,the degree of parental anxiety and understanding about neonatal jaundice knowledge were correlated with parental compliance(P<0.05).However,there was no significant correlation between maternal age,the way of delivery,gravidity,parity,parental nationality,the type of household and parental compliance(P>0.05).Conclusion The compliance of the parents with jaundice is not high.There are many influencing factors could reduce parental compliance.Including low household income,low level of education,the time it takes to long to come to hospital,the waiting time to long,parental anxiety,couldn't understand the knowledge of neonatal jaundice well,maternal complications during pregnancy,jaundice is serious,with complications,premature rupture of membranes,amniotic fluid pollution,placenta abnormality,umbilical cord abnormality,and fetal movement abnormality.Therefore,we could explore a more effective approach to communicate with patients and optimize the treatment process,so as to improve parental compliance and neonatal prognosis of jaundice.Part ? Effect of the animation demo on parental compliance of neonatal jaundice Objective To explore the effect of the animation demo on parental compliance of neonatal jaundice.Methods The knowledge of neonatal jaundice was made into cartoon animation video.923 neonates with jaundice who had jaundice symptoms during the treatment of jaundice were randomly divided into the anime communication group(457)and the traditional communication group(466).Traditional communication group doctors only communicated with their parents through oral interpretation and paper propaganda materials,and communicated with their parents about the neonatal condition.On the basis of the former,the animation communication group combines the cartoon animation video presentation with the parents to communicate with the newborns.A questionnaire was used to understand the correctness of the knowledge of neonatal jaundice and the compliance of the two groups of parents in the process of diagnosis and treatment.Results The correctness of the knowledge of neonatal jaundice and the compliance in the diagnosis and treatment process were better than those of the traditional communication group,the difference was statistically significant(P<0.05).Conclusions Animation demo of neonatal jaundice contributes to increase parents' medical compliance effectively,and helps for making doctors work smoothly.Part ? The application of animation demo in neonatal jaundice in the health education of Obstetrics Objective Discussion on neonatal jaundice knowledge animation demonstration method in our hospital obstetrics application,whether parents on neonatal jaundice knowledge,whether to increase,treatment compliance and neonatal pediatrician in doctor-patient communication on time and energy is reduced,whether to improve neonatal outcome.Methods All pregnant and lying in maternity and family members of Obstetrics Department of our hospital in January 1,2017 and August 31 st ~2017were selected as the research object,and they were randomly divided into observation group and control group.The observation group was taught by the animation demonstration method of neonatal jaundice.The questionnaire was used to understand the two groups of parents' understanding of neonatal jaundice,the compliance in diagnosis and treatment,the degree of difficulty in communicating with the patients and the prognosis of the newborns.Results In the observation group and the control group,487 cases of effective cases and 461 cases were collected,respectively.The parents' knowledge of jaundice in the observation group was better than that in the traditional group and the observation group.The compliance of the parents in the jaundice neonates was better than that in the control group,and the difference was statistically significant(P<0.05).The average time to communicate with the parents in the observation group was shorter than that in the control group(9.35 + 2.21 minutes vs12.57 + 3.15 minutes).The incidence of difficulty in communication between doctors and their parents in the observation group was lower than that in the control group(44.9% vs 59.1%),the difference was statistically significant(P<0.05).Compared with the control group,the duration of neonatal jaundice in the observation group was short(3.67 + 0.42 vs 5.11 + 0.45 d),the use of enhanced phototherapy and albumin was lower(14.2% vs 21.6%,7.1% vs 17.4%),the incidence of MRI abnormalities,unilateral or bilateral brainstem auditory evoked potential(BEAP)in the head were lower(3.5% vs6.4%,3.1% vs 5.7%),the time of hospitalization was shorter(5.36 + 1.12 vs 3.25 + 1.08d),and the difference was statistically significant(P<0.05).Conclusion The animation demonstration of neonatal jaundice can be used as a propaganda material in the Department of Pediatrics(New Pediatrics)and obstetrics department.It helps to ncrease parents' understanding of jaundice and the compliance of parents,reduce the workload of pediatricians,and reduce the risk of neonatal jaundice. |
