Analysis On Related Factors And Clinical Research Of Umbilical Cord Around The Neck

Objective To investigate related factors of nuchal umbilical cord,the impact of umbilical artery blood flow parameters(S/D ratio,RI value),stage of labor,delivery methods and perinatal prognosis on umbilical cord around the neck,relationship between fetal position,placental location,winding directions and winding angle.Methods 1.Collecting the pregnant women in 2010 who completed the entire pregnancy of the prescribed ultrasound exminnation in Guangxi Zhuang Autonomous Region people's Hospital and hospitalized childbirth with single fetus,2,109 cases in total.(1)11 possible factors in multivariable unconditional Logistic Model were chosen.(2)Divided into nuchal cord group and no nuchal cord group,retrospectively analyzed umbilical artery blood flow parameters(S/D ratio,RI value),stage of labor,delivery methods and the effects of perinatal prognosis.2.To select 679 cases of nuchal cord fetuses with complete records,analyzing the relationship between fetal position,placental position,winding directions and winding angle,Results(1)Age of pregnant women,the sex and cord length of the newborn have the significant effects on the cord around neck with one-way analysis.Multivariate analysis is showed that the longer the umbilical cord,the higher the incidence of umbilical cord around the neck;sex associates with umbilical cord around the neck.(2).Umbilical artery blood flow parameters(S/D ratio,RI value),stage of labor,delivery methods and perinatal prognosis,compared to the two groups have no statistical significance.(3)Most fetuses of umbilical cord around the neck is the ones with anterior wall of the placental attachment of uterus and most of them are C type and a type.Conclusion It shows the cord length is the major factor affecting the cord around the neck.In conditions of medical monitoring and immediate treatment measures,stage of labor,delivery methods and perinatal prognosis do not make too much sense for the nuchal cord.The angle of umbilical cord entanglement is determined by fetal position,placental location and winding direction formed by the interaction of the three.
