A Comparison Of The Treatment Effect On Type ? Cesarean Scar Pregnancy Of Curettage And TCRF

Objective:CSP is a dangerous long-term cesarean complication.Due to the two-child policy and the high cesarean section rate,CSP incidence was increasing year by year and it becomes common gynecologic emergency endangering female's life and health.There are kinds of treatment of CSP.Type I CSP is more common clinically,so this article focused on treatment of type I CSP.As we know,there were 2 surgical methods includes traditional curettage and TCRF.Due to the high bleeding risk and the convenience of TCRF,traditional curettage become gradually replaced by TCRF.But TCRF has its own disadvantage such as valuableness and bleeding,there is still no consensus.This article is aimed at Type I CSP,to compare the advantage and disadvantage of curettage and TCRF.Methods:Divide the 60 patients who had a CSP from Jan.2011 to Jun.2017 in the provincial 3-A-class hospital of into 2 groups,in which Section TCRF has 30 patients and Section CTG has 30 patients.All the qualified patients with stable vital signs and normal serum examine report.There were no obvious differences among the age,pregnancy time,cesarean delivery time,gestational sac diameter through TVS measurement,preoperative ?-hCG standard of the patients in 2 groups.Take notes of bleeding,the time on operation,the time in hospital,pain scores and costs,and time of menstruation recover,and ?-HCG recovery time of these patients.(All patients should take contraception method at least 3 months.And telephone follow-up study their menstruation recovery.)Results:It spends less time in operation(27.41±22.33 min to 45.20±20.04min),and in hospital(6.81±1.83 d to 10.61±5.01d),and it causes less pain(1.88±0.89 scores to 2.92±0.86 scores)and costs less(6179.70±2264.49 yuan to 10859.26±957.28 yuan)for patients to take curettage operation under the monitor of hysteroscopy.But on the other hand,it is same to TCRF in the respect of bleeding in operation(60.61±146.95 ml to 71.54±64.40ml),decreasing of Hb(18.06±8.21g/l to 16.69±11.11g/l),time of menstruation recover(31.82±4.52 d to 30.61±2.84d)and the time of patients' ?-HCG recover(34.51±4.54 d to 35.40±4.01d).Conclusion:Curettage under the monitor of hysteroscopy operation has its own advantages in the treatment to type I cesarean scar pregnancy than TCRF.It spends less time,costs less,causes less pain,while it can result to same effect: such as bleeding,menstruation recover and the ?-HCG recover.
