Viatorr Stent And Double Stent Technology In The Clinical Efficacy Of TIPS Surgery

Purpose: To observe the clinical efficacy of Viatorr stent and double stent technique for transjugular intrahepatic portosystemic shunt in the treatment of portal hypertension complications,and to investigate the effectiveness and safety of the two groups of stents.Method: 62 consecutive patients with portal hypertension underwent a TIPS procedure in Viatorr stent and double stent grafts between March 2015 to December 2016,The main symptoms were variceal bleeding(n=55)and refractory ascites(n=7).The follow-up time was 6 to 17 months,the median follow-up time was 11.2 months and 10.9 months,observed in patients with surgical complications and clinical efficacy.The t-test was used to analyze whether the changes in portal vein pressure before and after surgery were statistically significant.Result: Sixty-two patients with liver cirrhosis and portal hypertension underwent TIPS surgery.Totally 62 patients were successfully operated.A total of 22 Viatorr covered stents,40 bare stents,and 40 Fluency stent grafts with a diameter of 8 mm were placed.The portal venous pressure in Viatorr stent group and double stent group decreased from(38.4 ± 6.6)mm H2 O and(36.8 ± 5.8)mm H2 O to(25.2 ± 6.4)mm H2 O and(25.4 ± 5.3)mm H2 O before surgery,(t= 9.97,13.51,P<0.01).During the follow-up period,55 patients were bleeding after surgery stopped,7 patients with refractory ascites subsided.Viatorr double stents and stent patency rate was 90% and 95.4%,hepatic encephalopathy 9% and 15%,mortality during follow-up of 12.5% and 5%,There was no significant difference between the two groups.Conclusions: 1)TIPS surgery provides a minimally invasive and highly effective treatment for patients with cirrhosis.It has a wide range of application prospects in the treatment of portal hypertension complications.The use of Viatorr stent or double stent technology maintains smooth flow channels and reduces complications after TIPS.The occurrence and improvement of patient quality of life are of great significance,but the long-term efficacy of the domestic patient population remains to be further observed.2)The application of Viatorr stent in domestic TIPS surgery is still ahead of the curve,which solves the problem of stent compliance when double stenting is performed.There is no significant difference in the short-term efficacy of establishing a TIPS intrahepatic shunt with a Viatorr stent or a double stent.When the Viatorr stent is not widely used in clinical practice,double stent technology can be used as an alternative treatment.The long-term efficacy of Viatorr stents versus double stents requires further study.
