A Study About The Island Pectoralis Major Musle Flap In Rapairing Of The Defects After The Laryngeal Function Recerved Operation Of Hypopharyngeal Carcinoma

Backgroud:The position of head and neck is very special andthe anatomy is complicated.The repair of the defect after thehead and neck tumor surgery is always a difficultproblem.hypopharyngeal carcinoma is one of the high incidencehead and neck cancer,because of the particularity of its place ofconcealment and anatomic areas,Postoperative repair isparticularly important,Maximizing to keep fuction of the larynxis also one of the problems to be solved,It is related to thepatient's postoperative swallowing function and the recovery oflaryngeal function.In view of the anatomical position ofhypopharyngeal cancer,after operation,the pectoralis majormyocutaneous flap can be used to repair the defect well.At homeand abroad,it is very early to suggest that the pectoralis majormusculocutaneous flap can be used to repair the defect ofhypopharyngeal carcinoma.However,there is no relevantliterature on the application about pedicle island majormyocutaneous flap on keeping laryngeal function of hypophargealcarcinoma in one-stage repair.Objective:To explore the application about pedicle islandpectoralis major myocutaneous flap on Keeping laryngealfunction of hypopharyngeal carcinoma in one-stage repair.Methods: A retrospective analysis about 16 patients who gethypopharyngeal carcinoma surgery by using pedicle islandpectoralis major myocutaneous flap to repair,including 14 caseswhose primary tumors located in the piriform fossa,and 2 casesin pharynx posterior wall,All of the patients had squamous cellcarcinoma,and all patients had been kept laryngeal function.Results: In 16 cases,the blood supply of the thoracicmusculocutaneous flap was controlled by the acromial artery.allpatients were discharged from the hospital,all 16 cases' pectoralis major muscle flap were to survive,2 of them appearedto have a pharyngeal fistula,but recovered after transintion.Ofthe 16 cases,14 cases were removed the trachea tube in 1 ~ 3months,2 cases with tube survival,and all patients were satisfiedwith their appearance and the function.In 16 cases,2 of themsurvived 3 and a half years,dying of lung metastasis;One casesurvived for four years and died of local recurrence of tumor.Onewas four and a half years old and died of lung metastasis.Onecase survived four years,died of heart disease,and the other 11 survived until now.Conclusion: The pedicle island pectoralis major myocutaneousflap is very important in repairing the defect of head and necktumor,and the success rate of the repair of hypopharyngeal cancer is very high.the application in the surgical defect repair ofpharyngeal carcinoma with pedicle island pectoralis majormyocutaneous flap is safe and effective.
