Objective To evaluate the clinical diagnostic value of medical thoracoscopy in the unexplained pleural effusion and the morphological features of tuberculous pleurisy under medical thoracoscopy examination.Methods From May 2015 to December 2016,49 cases of patients in our department with unexplained pleural effusion were analyzed retrospectively,together with analysis of the characteristic phenotypes of tuberculous pleurisy under medical thoracoscopy examination and the results of pathological biopsy,ADA and CEA.Results In the 49 patients with pleural effusion of unknown causes,46 cases were definitely diagnosed(93.88%),of which 29 cases were tuberculous pleural effusion(93.55 %).Tuberculous pleurisy was mainly divided into six different types: pleural diffuse hyperemia edema(20 cases),miliary nodules(16 cases),cellulose like adhesion(15 cases),capsulated effusion(7cases)and caseous necrosis(4cases).There was no significant difference in the expression of caseous necrosis between the tuberculosis group and the malignant group.Conclusion Medical thoracoscopy has a higher diagnostic value for unexplained pleural effusion,especially for difficult to diagnose tuberculous pleural effusion,and tuberculous pleurisy have characteristic morphological phenotypes under medical thoracoscopy examination. |
