Background and objective Wellens syndrome is a condition in which some patients with unstable angina can have T-wave inversion in chest pain and no significant ST change,or the original T-wave inverted deepening or becoming upright,but the original T-wa ve after angina is relieved and then further symmetric inversion or bidirectional,with varying duration from one hour to several weeks.This clinical phenome non is called Wellens syndrome.The cause of this correction is mainly caused by severe proximal descending coronary stenosis,so some scholars call it left descending T-wave syndrome.The specific cause and mechanism are not yet fully understood,but wellens syndrome is likely to turn into extensive anterior wall myocardial infarction,acute left heart failure,even death and other life-t hreatening conditions.Therefore,it is important to raise awareness of the chan ges in the electrocardiogram(ECG)T wave in Wellens syndrome and to provid e targeted treatment,otherwise there will be serious consequences.This article aims to further understand the clinical characteristics of Wellens syndrome thro ugh the analysis of clinical cases and electrocardiograms,to provide ideas for clinical diagnosis and treatment.Method 20 patients with wellens syndrome who were enrolled in the First Affiliated H ospital of Anhui Medical University from January 2016 to December 2017 wer e selected as the study subjects.Correlation results were statistically analyzed b y collecting medical history and electrocardiogram,removing contraindications a nd applying stent.ResultA total of 20 patients were included,aged 58-75 years,average age(66.7±3.63)yea rs);Of these,15 were males and 5 females;Angina pectoris course 2 months-3 years;There were 18 cases of hypertension,7 cases of diabetes mellitus and 20 cases of hyperlipidemia.All patients had angina pectoris history,the avera ge duration was(13.15±3.84)min,electrocardiogram analysis revealed 12 cases of type I,5 cases of type II,and 3 cases of T-wave organ-like change.The re sults of coronary angiography confirmed that there were anterior descending les ions,mainly in the proximal to midsection stenosis.Among all the lesions,Th ere were 14 cases with lesions>90%,4 cases with lesions between 70% and 89%,and 2 cases with lesions between 50% and 69%.Conclusion1.Wellens syndrome is characterized by T-wave characteristic changes in electro cardiogram(ECG)chest leads during the intermittent period of angina pectoris,mainly at V2-V4 leads,mostly due to severe stenosis in the proximal to midse ction of the anterior descending branches,while blood pressure,blood sugar,bl ood lipids,age and gender are all risk factors for Wellens syndrome.2.The change of T Bote characteristic of electrocardiogram is very important to the diagnosis of wellens syndrome.In particular,the change of T wave in ele ctrocardiogram may be a type of the change of T wave of wellens syndrome.
