Background and Objective :Pulmonary sarcomatoid carcinoma is a relatively rare form of non-small cell lung cancer that contains both epithelial and sarcomatoid interstitial components and is predominantly sarcomatoid,accounting for about 0.5% of NSCLC,tumor progression is rapid,easy to relapse,the traditional radiation therapy and chemotherapy are not sensitive.At present,there are few researches on the pulmonary sarcomatoid carcinoma and the available treatment modalities are limited,which has become a major challenge in clinical treatment.So finding suitable biomarkers to predict the prognosis of pulmonary sarcomatoid carcinoma patients and further exploring novel treatment modalities that patients may benefit from is crucial.Several studies have shown that PD-L1 and Rab1 A can promote tumor proliferation,progression and metastasis,both were significantly associated with mTOR activation.However,the expression of PD-L1 and Rab1 A in pulmonary sarcomatoid carcinomas is not yet fully studied at home and abroad.This study aims to detect the expression of PD-L1 and Rab1 A in patients with pulmonary sarcomatoid carcinoma by immunohistochemistry,and to investigate the influence of the expression of both on the prognosis of pulmonary sarcomatoid carcinoma.Materials and methodsWe collected the paraffin specimens of pulmonary sarcomatoid carcinoma issune from 44 patients and control group(lung squamous cell carcinoma,adenocarcinoma)from 45 patients in the First Affiliated Hospital of Zhengzhou University and the Second Affiliated Hospital of Zhengzhou University,the immunohistochemistry staining was used to examine the expression of PD-L1 and Rab1 A in two groups of cancer tissues.PD-L1 with a median of 10% as the critical point,?10% as high expression,<10% as low expression,Rab1 A with immunohistochemical staining score median score of 170 as the critical value was divided into high expression group and low expression group.Data were processed using SPSS 18.0 statistical analysis software.The measurement data between the two groups were compared by t-test,and the comparisons between groups were analyzed by chi-square test.The univariate survival analysis of categorical data was compared using Kaplan-Meier test and Cox proportional hazards regression model was used to analyze and compare factors affecting prognosis.Test level ? = 0.05.Results1.Patients with high expression of PD-L1 in pulmonary sarcomatoid carcinoma and control group(lung squamous cell carcinoma and adenocarcinoma)were 28 cases and 14 cases respectively(63.64% VS31.11%,P<0.05),Rab1 A high expression group in pulmonary sarcomatoid carcinoma and control group(lung squamous cell carcinoma,adenocarcinoma)were 30 cases and 21 cases respectively(68.18% VS46.67%,P<0.05).There was no significant correlation between the expression of PD-L1 and Rab1 A and the patient's age,gender,smoking history,tumor site and clinical stage(P>0.05).2.The expression of PD-L1 and Rab1 A had no obvious correlation.3.The median OS of patients with high expression and low expression of PD-L1 in pulmonary sarcomatoid carcinoma was 9 months and 13 months respectively,the difference was statistically significant(P<0.05).The median OS of patients with high expression and low expression of Rab1 A in pulmonary sarcomatoid carcinoma was 9 months and 14 months respectively,the difference was statistically significant(P<0.05).4.Multivariate Cox proportional hazards regression analysis showed that age(P=0.011),clinical stage(P=0.039),surgical treatment(P<0.001),and PD-L1 expression(P=0.035)were independent influencing factors of OS in patients with pulmonary sarcomatoid carcinoma.Conclusions1.High expression of PD-L1 and Rab1 A in pulmonary sarcomatoid carcinomas compared to lung cancer controls,there was no significant correlation between PD-L1 and Rab1 A expression in pulmonary sarcomatoid carcinoma.2.The age,clinical stage,surgical treatment and PD-L1 expression were independent influencing factors of OS in patients with pulmonary sarcomatoid carcinoma.
