Objective:To analyze the effect of using negative pressure vacuum Cushion and foot fixator in the treatment of lower extremity cancer in radiotherapy positioning.Methods:86 cases of lower limb tumor patients admitted in our hospital from June 2012 to September 2016 were randomly divided into A and B groups.During the radiotherapy,grouPA was fixed with a foot holder,grouPB was fixed with a foot holder and a negative pressure vacuum pad.The patients were selected with the supine position,and the hands were crossed and the body was relaxed and the body was relaxed.The whole process was The patient patients are located in a more comfortable location and state,and according to the requirements of precise positioning and fine scan.The differences of the placement,the adverse reactions were compared between the two groups before and after the different fixation methods.Results:In the position error,the error of the placement in the single grouPwas<2mm in the single direction,the placement error of the grouPA was:front and back(0.26 ±0.192cm),the left and right(0.29 ± 0.112cm),the upper and lower(1.22 ±0.543cm);the placement error of the grouPB was:front and back((0.13 ±0.093cm),left and right(0.16±0.141cm),uPand down(0.67±0.531cm).The error of the three positions of grouPB was significantly less than that of grouPA(P<0.05).The error of placement in the upper and lower directions of the two groups was significantly larger than that of the left and right direction,and the error of the left and right direction was significantly higher than that of the control group,the difference was statistically significant(P<0.05).In the target area by the dose distribution and fitness index:There was no significant difference between D95,D50 and Dmean in the two groups(P>0.05).The V100 was(98.65 ± 1.15)%in grouPB,(95.03 ± 1.24)%in grouPA,B grouPwas significantly higher than the A group,the difference was statistically significant(P<0.05);The conformal index(0.78 ± 0.08)in grouPB was significantly better than that in grouPA(0.42 ± 0.03),and the difference was statistically significant(t= 27.63,P= 0.00).In the case of endangering organ exposure:The V30 and V5 of grouPB was(6.21±1.18)%?and(17.14±3.08)%,The V30 and V2 of grouPA was(6.85±1.28)%?and(21.20±7.06)%,B grouPV30,V5 were significantly lower than the A group,the difference was statistically significant(P<0.05).GrouPB patients D30,D50 and Dmax were significantly lower than the control group,and the difference was statistically significant(P<0.05).In terms of adverse reactions:The total incidence of adverse bone tissue reactions in grouPB(37.21%)was significantly lower than that in grouPA(60.47%),and the difference was statistically significant(P<0.05);The total incidence of adverse skin reaction(34.88%)in grouPB was significantly lower than that in grouPA(58.14%),and the difference was statistically significant(P<0.05).Conclusion:The use of negative pressure vacuum pad and foot fixator in the treatment of lower extremity cancer patients have a significant fixation effect,the two combined effect is more significant,can effectively reduce the patient's lower extremity tumor radiotherapy before and after,about The orientation error in the three-dimensional direction is more than 2mm in the single direction,which obviously improves the dose distribution of the target area and reduces the dose of the surrounding normal tissue,and the higher the conformational index,Endanger the organ dose was significantly reduced,thereby reducing the surrounding normal tissue adverse reactions.The use of negative pressure vacuum pad and foot fixation in radiotherapy to fix the patients with lower limb tumor is significant and deserves to be popularized clinically.
