ObjectivesUnderstanding the status of life quality and emotional disorders of HFmrEF patients,and analyzing the influencing factors of the former provide scientific basis for clinical nursing,improve the quality of life,reduce the mental obstacle and improve the overall nursing level of the patients.MethodsIn this study,a total of 219 HFmrEF patients were selected from December 2016 to October 2017 in the Department of Cardiology of a three grade first-class hospital in Kaifeng City.The research tools included the general data questionnaire,the Self-Rating Anxiety Scale(SAS),the Self-Rating Depression Scale(SDS)and the Minnesota Living with Heart Failure Questionnaire(MLHFQ).The data were analyzed by SPSS22.0 statistical software.The descriptive statistics of the basic cases of the patients were analyzed by percentage,frequency,mean and deviation etc.The impact factors were analyzed by independent sample t test,one-way ANOVA and multiple linear stepwise regression analysis.Results1 Basic status of life quality of HFmrEF patientsThe total score of life quality was(60.18±20.45).The average score of each dimension from high to low was in the emotional field(51.98±21.14),body field(32.46±10.67),and other areas(17.24±9.72).2 One-way ANOVA of life quality of HFmrEF patientsIn the One-way ANOVA,the scores of life quality of HFmrEF patients no elder than 65 were higher than those elder than 70 years,and the differences in the body,emotional and other areas in different age groups were statistically significant(P<0.05).The scores of all dimensions of life quality of married HFmrEF patients were higher than those of non married people(P<0.05),so are the patients with different times of hospitalization.The scores of BMI for overweight and obesity were higher than those of normal and emaciated persons(P<0.05),and those with normal BNP levels were higher than those of non-normal(P<0.05);the HFmrEF patients with LVEF in 43 to 45%,46 to 49% were higher than those in 40 to 42%(P<0.05).3 Correlation between BNP,LVEF and BMI and quality of life in heart failure patients with intermediate ejection fractionSpearman correlation analysis showed that the scores of BNP and HFmrEF were negatively correlated(r=-0.686,P<0.05),and there was a positive correlation between LVEF and the life quality score of HFmrEF patients(r=0.486,P<0.05),and there was a positive correlation between BMI and HFmrEF patients' quality of life(r=0.488,).4 The factors of statistical significance in One-way ANOVA were taken as independent variables,and the total score of life quality of HFmrEF patients was used as a dependent variable,and multiple linear stepwise regression analysis was carried out.The results showed that the factors influencing the quality of life were age,marital status,number of hospitalization,BMI,BNP and LVEF(P<0.05).5 Basic status of emotional disorder of HFmrEF patientsAmong the HFmrEF patients,there were 110 patients with mood disorders,accounting for 50.7%.Concretely,there were 68 cases(79.1%)with emotional disorders in patients whose ejection fraction values were from 40 to 42%,27(36%)from 43 to 45%,15(25.4%)from 46 to 49%.6 One-way ANOVA of emotional disorders of HFmrEF patientsIn single factor analysis,the incidence of emotional disorders in women was higher than that of men(P<0.05);the incidence of emotional disorders in HFmrEF patients with low level of education was relatively high(P<0.05);the incidence of emotional disorders in HFmrEF patients with different occupations was also significantly different(P<0.05);the incidence of BMI for normal and thin emotional disorders was higher than that of overweight.And obese HFmrEF patients(P<0.05);the incidence of emotional disorders in anemia HFmrEF patients was higher than that of normal persons(P<0.05);the incidence of emotional disorders in HFmrEF patients was higher than that of those with normal bilirubin level(P<0.05);the incidence of LVEF in 40% to 42% of emotional disorders was higher than that of HFmrEF patients with 46% to 49%(P<0.05).7 The correlation between BNP,total bilirubin,anemia,BMI and LVEF and emotional disorder of HFmrEF patientsSpearman correlation analysis showed a positive correlation between BNP and HFmrEF patients(r=0.517,P<0.001);total bilirubin was positively correlated with emotional disorders in HFmrEF patients(r=0.359,P<0.001);BMI and HFmrEF patients were negatively correlated with emotional disorders(r=-0.342,P<0.001);anemia and emotional disorders were positive and negative phase(r=-0.443,p<0.001);left ventricular ejection fraction was negatively correlated with emotional disorder in HFmrEF patients(r=-0.500,P<0.001).8 Two classification logistic regression analysis of emotional disorders in HFmrEF patientsIn the One-way ANOVA,the variables with statistical significance were used as independent variables,and whether the patients with HFmrEF produced mood disorder as the dependent variable,and multivariate linear regression analysis was performed.Results show that the ejection fraction in patients with intermediate value factors influencing the quality of life for age,gender,educational level,occupation,left ventricular ejection fraction,BMI,BNP,anemia,total bilirubin level,(P<0.05).9 The correlation between emotional disorder and life quality in HFmrEF patientsSpearman correlation analysis showed that there was a negative correlation between emotional disorder and quality of life in HFmrEF patients,and the correlation coefficient(r=-0.525,P<0.001).Conclusions1 The life quality of HFmrEF patients was poorer,and the influence factors mainly include gender,educational level,occupation,BMI,anemia,total bilirubin level,LVEF.2 BNP,BMI and LVEF were correlated with the life quality of HFmrEF patients.3 The life quality of HFmrEF patients was in the middle and lower levels,and the main influence factors were age,marital status,BMI,BNP,hospital admission,LVEF.4 BNP,total bilirubin,anemia,BMI and LVEF were correlated with the emotional disorder of HFmrEF patients.5 There was a negative correlation between emotional disorder and life quality of HFmrEF patients.The lighter the anxiety and depression of HFmrEF,the better their living conditions.Nursing staff should consider the influence factors affecting the quality of life of HFmrEF patients and formulate specific intervention measures in order to improve the life quality of HFmrEF patients,and to communicate effectively with the patients' own characteristics of HFmrEF,and to provide corresponding psychological nursing and improve the patient's bad mood.
