Objectives1.To analyze the level and influence factors of Posttraumatic Growth among young and middle aged stroke patients.To construct an empowerment based intervention for young and middle-aged stroke survivors.2.To explore the effect of empowerment based intervention on young and middle-aged stroke survivors.MethodsThis study was divided into two parts:1.172 stroke patients were recruited from five hospitals in Zheng Zhou,they were pleased to sign the general questionnaire,Chinese Posttraumatic Growth Inventory and stroke Self-efficacy Questionnaire,Barthel Index,using Pearson correlation and multiple stepwise regression analysis to explore the relationships between SSEQ and the other variables.Based on literature review and finished study,5 patients with stroke were taken a pilot investigation,then conduct the specialist consultation,after doing this,form an intervention project finally.2.72 young and middle-aged patients with stroke were divided into control group(n=38)and intervention group(n=34).The control group adopted the routine rehabilitation education and the two interventions themes of empowerment based intervention the intervention group received empowerment based intervention.It last for 6 weeks,Barthel Index Scale(BI),Stroke Self-Efficacy Questionnaire(SSEQ)and Chinese Posttraumatic Growth Inventory(C-PTGI)scores in stroke patients were compared to evaluate the baseline and intervention effects.Results1.Regression analysis showed that the self management dimension of rehabilitation self-effiency was important factors for the stroke survivors'PTG.It had8 themes and 6 weeks intervention dosage which was conducted by rehabilitative therapist and researcher,the themes were health guidance,self management,positive coping,inspiring gratitude,reshaping hope,preventing recurrence,social support and summarizing prospects.2.In this intervention phase,83 young and middle aged stroke patients were recruited,during the intervention,11 cases were lost,the rate of loss was 13.25%.there were 72 effective samples.The age of young and middle aged stroke patients was 20~64 years old.(1)Using repeated-measures analysis to explore the results of the SSEQ snd two dimensions.It showed that there were statistically significant in the SSEQ snd self management dimension between groups,time effect and interaction effect(P<0.05).(2)Using repeated-measures analysis to explore the results of the C-PTGI and five dimensions.It showed that the difference were statistically significant in the C-PTGI between groups(F=10.471,P=0.002)and time effect(F=64.102,P<0.001),and there was interaction effect between time and group(F=5.548,P=0.018).There were statistically significant in the dimensions of appreciation of life,new possibilities,the relationships with others,personal strength between groups,time effect and interaction effect(P<0.05).(3)The repeated-measures analysis of the BI scores indicated that the difference were no statistically significant between group effect(P=0.192),but time effect(F=36.044,P<0.001),and there was interaction effect between time and group(F=4.492,P=0.029).The generalized estimation equation showed that the dependence degree of patients had statistical difference with time(Wald~2=49.642,P<0.001).Conclusions1.It was suggested to take actions to enhance the self-efficacy in young and middle aged stroke patients so as to improve the rehability activities of the young and middle aged stroke patients.2.The implementation of empowerment based intervention could promote the self-efficacy,improve Posttraumatic Growth and ADL in young and middle-aged stroke patients.The intervention might provide valuable reference for young and middle-aged stroke patients to excavate their own rehabilitation potential.
