Research objectives:This study investigated the ultrasonographic features of scar pregnancy,divided the cases of scar pregnancy diagnosed by ultrasound into groups with different degree of risk according to the ultrasonographic features,constantly monitored the occurrence of early secondary placenta implantation during early pregnancy(before 12 weeks of gestation),analyzed the clinical outcome of different groups and the relationship between early secondary placenta implantation and the clinical outcome,and thereby,provided the basis for the natural process of the scar pregnancy and individualized post clinical treatment plan.Research method:1.The information of 32 cases of scar pregnancy from August 2016 to March 2018 in the Department of ultrasound,Fifth Affiliated Hospital of Zhengzhou University was retrospectively collected.2.Combined with the ultrasound data,clinical examination and pathological data collected from the suspected scar pregnancy to different gestational weeks,the 32 cases of scar pregnancy were divided into high-risk group,middle risk group and low risk group according to the ultrasonographic features,and the clinical outcomes of the three groups were investigated.3.The cases suspected to be scar pregnancy were continuously observed in the late process of development(within 12 weeks of pregnancy)through ultrasound imaging in order to diagnose whether an early secondary placental implantation happened.4.The general information of different groups,including age,the age of primary cesarean section,the number of cesarean section,the interval time from the last cesarean section,the time of menopause,and the HCG value obtained at the first ultrasound diagnosis in our hospital,was compared.Research results:1.The differences among the general information of different groups of scar pregnancy,like the number of cesarean section,the interval time from the last cesarean section,the number of menopause days,and the HCG value obtained at the first ultrasound diagnosis in our hospital,were not statistically significant.2.Among the 32 suspected cases of scar pregnancy,14 cases were clinical treated because of the discontinuation of embryonic development or abortion factors in 12 weeks.The pathological results confirmed that 13 cases of them were scar pregnancy and 1 case was cervical tube pregnancy.3.Among the 32 suspected cases of scar pregnancy,11 cases were indicated early placental implantation secondary to scar pregnancy according to early ultrasonic images(in 12 weeks),and 13 cases of placental implantation were confirmed by post pathological or clinical evidence.Conclusion:1.According to the characteristics of ultrasonic imaging,the patients with scar pregnancy can be grouped and their clinical outcome can be predicted.2?Through the combination of transvaginal ultrasound and transabdominal ultrasound,the placenta accrete in patients with scar pregnancy can be diagnosed during early pregnancy.
