Objective:The study aims to compare the efficacy of arterial infusion chemotherapy and systemic chemotherapy in the treatment of esophageal squamous cell carcinoma(ESCC).Methods:A total of 33 patients with ESCC were selected and divided into two groups randomly.The systemic chemotherapy group contained 18 cases.Patients with squamous cell carcinoma were treated with DCF regimen(Docetaxel 75mg/m2、Lobaplatin 30mg/m2、5-fluorouracil 1000mg/m2)for 3 weeks a cycle,3cycles a course of treatment.15 cases in the arterial infusion chemotherapy group used arterial infusion chemotherapy treatment combined with spiral CT 3D angiography,It was the same with systemic chemotherapy in choosing treatment prescription,but different from the dose which was half of the systemic chemotherapy group.Drugs(DCF)were dissolved in saline(50ml),the injection time was more than 15min for 1 time 3 weeks and a course of treatment contained 3 times.Both of the patients in this two group have done gastric barium examination regulary in every course of the treatment cycle.Tumor remission degree were evaluated according to tumor size and clinical symptoms.Complete response(CR):No significant changes in tumor size,symptoms and signs for at least 4 weeks;Partial response(PR):Tumor reduction greater than 50%at least 4 weeks;Stable disease(SD):No significant changes in the condition,tumor size Increased<25%or reduced<50%;Progress disease(PD):New lesions appearred or the original lesion increased≥ 25%Results:1.The degree of tumor remission in perfusion chemotherapy group CR is 4 cases and PR is 9 cases,the total effective rate(CR+PR)is 86.7%.In the systemic chemotherapy group,2 cases were CR and 4 cases were PR.The total effective rate was 33.33%.There was a significant difference between the two groups(P<0.05).2.Compared with the perfusion chemotherapy group and the systemic chemotherapy group,there were 5 cases(33.33%)and 15 cases(83.33%)of gastrointestinal reaction;5 cases(26.67%)and 14 cases(77.78%)of myelosuppression;4 cases(26.67%),and 15 cases(83.33%)of liver and kidney dysfunction,and there were statistically significant differences(all P<0.5).The success rate was 100%in the arterial infusion chemotherapy group.There was no fever,chest pain,cough,vomiting,hemoptysis,spinal cord injury,perforation of the esophagus,necrotizing esophagitis,and so on.Conclusion:1.The treatment efficacy is better and adverse reaction rate is less using perfusion chemotherapy than systemic chemotherapy in the esophageal squamous cell carcinoma2.Using CTA before perfusion chemotherapy treatment can improve the success rate and also reduce the incidence of side effects...
