Objective:To investigate the application value,feasibility and safety of3.0T magnetic resonance IVIM imaging in assists CT guided percutaneous lung biopsy.Methods:This study included patients who were suspected lung cancer in our hospital from June,2017 to February,2018.The patients were randomly divided into group A and group B.In group A,patients were underwent CT enhancement scanning and 3.0T MRI IVIM scanning.Percutaneous lung biopsy were performed under CT-guided by two radiologists who reviewed the CT enhanced images,DWI images and ADC pseudo color images?including Standard ADC and Slow ADC?.In group B,percutaneous lung biopsy were performed under CT-guided by two radiologists who just reviewed the CT enhanced images.The true positive rate and complications of biopsy between two groups were analysed.Results:In group A,62 patients were collected,and 248 patients were collected in group B.There were no statistically significant?P>0.05?between group A and B when compared the gender,age,size and emphysema of the two groups;In group A,there were 52 cases of malignant lesions and 10benign lesions;and in group B,there were 212 cases of malignant lesions and36 benign lesions,there was no statistically significant?P>0.05?between constituent ratio of the two groups;In group A,ADC and D values of the malignant lesions were?1.453+0.164?*10-3mm2/s??0.846+0.186?*10-3mm2/s,ADC and D values of benign lesions were?1.809+0.238?*10-3mm2/s??1.356+0.340?*10-3mm2/s,There were significant difference in ADC and D values between benign and malignant lesions?P<0.05?;The D*value and f value of malignant lesions in group A were?16.527+10.637?*10-3mm2/s??25.569+14.332?%,D*value and f value of benign lesions were?23.343+26.334?*10-3mm2/s??31.475+14.585?%,There were no significant difference in the value of D*and f between benign and malignant lesions?P>0.05?;The area under the ROC curve of ADC value,D value,D*and f value were 0.899,0.926,0.619,0.670.The positive rates of group A and B were 98.38%?61/62?and 89.11%?221/248?respectively,and the difference between the two groups was statistically significant?P<0.05?.There were 8cases of complications in group A,and 42 cases in group B,includ pneumothorax,bleeding etal.The complication incidence rate of group A and B was 12.90%and 16.94%respectively,and there was no significant difference between two groups?P>0.05?.Conclusions:ADC value and D value of IVIM imaging are helpful for the differentiation of benign and malignant lung lesions;3.0T magnetic resonance IVIM imaging combined with CT-enhanced scanning can improve the positive rate of percutaneous lung biopsy.This method is safe and feasible,and has important clinical significance.
