Background:Inflammatory bowel disease is a group of intestinal inflammatory diseases whose pathogenesis is not clear.The diagnosis of inflammatory bowel disease often needs to be combined with endoscopy,clinical manifestations and histopathological manifestations.It may be that intestinal bacteria and environmental factors play an important role in the maintenance of intestinal function in people with genetic susceptibility to immunoreactive diseases.And intestinal barrier plays an important role in the maintenance of intestinal function.Damage to the structure and function of tight connections can lead to increased intestinal permeability.The tightly connected components mainly include the atresia protein(Occludin)and the Claudin protein.Part I: clinical,endoscopic and pathological features of inflammatory bowel diseaseObjective:To analyze the clinical,endoscopic and pathological characteristics of IBD.Method:Clinical,endoscopic and pathological characteristics of 99 patients with IBD[including 61 ulcerative colitis(UC)and 38 Crohn's disease(CD)] from January 2001 to April 2017 at Yijishan Hospital were retrospectively analyzed.Result:Compared with CD patients,age in UC patients was significantly increased,incidences of diarrhea,bloody stool were significantly increased while incidence of abdominal mass was significantly decreased(P<0.05).Extensive type UC and ileocolonic type CD were common.The main pathological features of UC were cryptic branching(57.1%),cryptic twisting(76.2%),cryptic atrophy(54.8%)and cryptic irregularity(28.6%).Ulcer was seen in 78.9% of CD patients with active chronic inflammation(mainly lymphocyte and plasmocyte infiltration),and detection rate of non-caseating granuloma was 15.8%.Conclusion:UC patients were characterized older age,diarrhea,bloody stool and endoscopic features were mucosal bleeding and ulcers.Pathological characteristics of UC were mainly cryptic architectural change and active chronic inflammation,while those of CD were mainly chronic inflammation with lymphocyte and plasmocyte infiltration and non-caseating granuloma.The depth of UC lesions was basically confined to the mucous layer,while that of CD was deeper.Part ?: Intestinal permeability in the pathogenesis of inflammatory bowel diseaseObjective:Study on intestinal permeability in inflammatory bowel diseaseMethod:(1)The IBD patients(including CD and UC patients)and normal control group were selected.The colonoscopy biopsy tissue of the IBD patients and normal control groupwas were selected.The expression and distribution of Claudin-1 and Occludin were detected by immunohistochemistry.(2)TNBS-induced acute inflammatory bowel disease model in mice was established.The histopathological change of colon tissue in experimental group and control group was observed using HEstaining.Western blot method and immunohistochemistry method were used to detect the expression and score of tight junction protein Claudin-1 Occludinin in intestinal tissue of mice.PCR method was used to detect the m RNA expression of Claudin-1 and Occludin in intestinal tissue.Result:(1)The expression of Claudin-1and Occludinin in normal control group was significantly higher than that in CD group and UC group(P < 0.05).The expression of Claudin-1 and Occludin in intestinal mucosa was expressed at the apical junction of the cell and distributed along the cell membrane.(2)Compared with the control group,the mice in the model group appeared to be eating and weight loss,loose fur,unresponsive.(3)The control group of mice observed in colonic mucosa of pink,model group observed lesions of the colon,deformation,partial lumen stenosis,twist stiffness,the length of the control group;the pathological observation of colon mucosa intact,without ulcer,continuous mucosa is complete,gland body arrangement rules,clear structure,mucous membrane and mucous membrane submucosa,lamina propria and muscular layer of normal model group,pathological observation of colonic lamina propria inflammatory cell infiltration,goblet cells,epithelial cells decreased,collagen increased,broken crypt disappeared,inflammation score and histological grading were significantly higher(P<0.05).(4)Compared with the control group,the expression of Claudin-1 and Occludin protein in colonic tissue of model group decreased significantly(P < 0.05),and the m RNA expression of Occludin and Claudin-1 was also decreased.Conclusion:(1)the expression of intestinal permeability related protein may be involved in the formation of inflammatory bowel disease.(2)the animal colitis model can be induced by TNBS enema.
