The treatment of cerebral infarction mainly includes venous thrombolysis?antiplatelet aggregation?anticoagulation?fiber?expansion and neuroprotection.If the onset time is less than the treatment time window,the arterial thrombolysis and stenting can be considered.Treatment time window and treatment cost limit,many patients cannot get the optimal treatment.For more than thrombolytic therapy time window or restricted by economic condition could not accept patients with Artery should,try line arterial Indwelling Catheter medication,through drug arterial Indwelling tube directly to the ischemic region.In this study,the therapeutic effects of arterial catheterization were analyzed by comparing the changes in the reference values of CT perfusion before and after treatment.Purpose:The therapeutic effect of arterial catheterization was compared by quantitative measurement of the changes of the reference value of CT head before and after treatment.Methods:Choose on January 1,2013 to January 1,2018 in the fellowship at China-Japan Union Hospital of Jilin University neurology cerebrovascular angiography confirmed unilateral internal carotid artery occlusion and collateral circulation open a total of 40 patients with cerebral infarction.The 40 patients who met the conditions were divided into two groups according to the different drug delivery channels: the control group received the routine treatment of the vein,and 20 cases were treated in total.A total of 20 patients were treated with femoral artery catheterization as observation group.Based on the results of the CT infusion of 40 patients,a quantitative measure of the before and after the treatment of the regional cerebral blood flow?regional cerebral blood volume?the time to peak mean and the transit time.By comparing the changes of each reference value before and after treatment,the therapeutic effect of the arterial catheterization was analyzed.Results:1.After treatment with arterial indwelling catheter,rCBV?rCBF?TTP and MTT were all better than before treatment.2.Ischemia Penumbra through arterial indwelling catheter,the rCBV and rCBF was increased than before treatment.The TTP and MTT was decreased,and improved perfusion core area is higher than the infarction.3.The improvement of rCBV?rCBF?TTP and MTT was not obvious after intravenous administration in ischemic central area.4.The improvement of rCBV and rCBF before and after the treatment of ischemic Penumbra zone was not obvious,and TTP and MTT were significantly improved.5.The improvement of blood perfusion in ischemic central area and ischemic Penumbra zone was more obvious than that of intravenous administration.Conclusion:1.The CT perfusion of patients with arterial indwelling catheterization and intravenous treatment was improved,and the perfusion improvement was more obvious in patients treated with arterial indwelling tube.2.Combined treatment with idaravone,ganglioside and danhong injection for acute cerebral infarction can improve blood perfusion at cerebral ischemia.
