Objective:To investigate the changes and possible role of estrogen and its receptor in the pathogenesis of primary gout arthritis.Methods:?1?The peripheral blood of 246 pGA?93 acute gout patients?AG?,98 interval gout patients?IG?,55 chronic gout patients?CG??,98asymptomatic hyperuricemia patients?AH?and 99 heathy controls?HC?were collected.The clinical data and laboratory indicators of them were enrolled.Plasma and peripheral blood mononuclear cells?PBMCs?were isolated from blood in vitro.?2?The expression of estradiol?E2?in serum was dectected by the Chemiluminescent Microparticle Immunoassay?CMIA?.?3?The expression of ER?,ER?and GPR30 mRNA in PBMCs was measured using Real time quantitative polymerase chain reaction?RT-qPCR?.?4?Spearman correlation analysis was used to analyze the level of plasma E2,ER?,ER?and GPR30 mRNA,and the correlation between the clinical and laboratory indexes of the AH group and the pGA group.?5?The expression of ER?,ER?and GPR30 protein in PBMCs was measured using Western blot?WB?.?6?Statistical Package forme Soci-science?SPSS?17.0 statistical software was used for analysis.The measurement data were compared by wilcoxon rank sum test or one factor analysis of variance test.The correlation between variables was used by Spearman correlation analysis.Results:?1?In plasma,AH,pGA,AG,IG group E2 levels were significantly lower than that of group HC?30.7±8.8pg/ml vs 30.9±8.3pg/ml vs 28.6±8.3pg/ml vs 31.4±8.3pg/ml vs 36.5±9.9pg/ml,P<0.05,respectively?,AG group was lower than that of IG group and CG group?28.6±8.3pg/ml vs 31.4±8.3pg/ml vs 33.8±7.3pg/ml,P<0.05,respectively?,AH group than in CG group?P<0.05?,and no significant difference between group HC and group CG difference?P>0.05?.?2?In PBMCs,the expression level of HC group ER?,and ER?and GPR30 mRNA?0.00024+0.00025,0.0047+0.0031,0.0020+0.0015?was significantly higher than that of group pGA?0.00015+0.00011,0.0036+0.0020,0.0013+0.0007?,AG group?0.00013+0.00011,0.0033+0.0014,0.0011+0.0008?and IG group?0.00019+0.00012,0.0040+0.0023,0.0014+0.0008?and CG group?0.00014+0.0001,0.0036+0.0022,0.0013+0.0005??P<0.05,respectively?,the expression level of ER?and GPR30 mRNA is higher than that of AH group?0.00015+0.00008,0.0014+0.0009??P<0.05,respectively?.The expression level of ER?mRNA in group AH?0.0044+0.0017?was higher than that in group pGA?P<0.05?,but there was no significant difference between group HC and HC group?P>0.05?.The expression level of ER?,ER?and GPR30mRNA in group AG was lower than that in group IG?P<0.05,respectively?,and the expression level of ER?and GPR30 mRNA was lower than that of AH group?P<0.05,respectively?.The expression level of ER?mRNA in group CG was lower than that in group IG?P<0.05?,and the expression level of ER?m RNA was lower than that of AH group?P<0.05?.?3?Spearman correlation analysis showed that the level of E2 was negatively correlated with ALT in pGA?r=-0.131,P=0.004?,ER?and ER?,GPR30 mRNA were positively correlated with ALB?r=0.226,P=0.000;r=0.177,P=0.005;r=0.141,P=0.027,respectively?,ER?was negatively correlated with AST,MO?r=-0.189,P=0.003 r=-0.150;P=0.018,respectively?.?4?In PBMCs,the expression level of HC group ER?and GPR30 protein?1.8+1.27,1.6+0.82?was significantly higher than that of group pGA?1+0.48,0.8+0.57?,AG group?0.5+0.39,0.5+0.47?,IG group?1.2+0.28,1+0.59?,CG group?1.3+0.38,0.9+0.61??P<0.05,respectively?.The expression level of AH group ER?protein?1.7+1.04?is higher than that of pGA group,AG group,IG group and CG group?P<0.05,respectively?,the expression level of GPR30protein?1.02+0.55?was significantly higher than that of AG group?P<0.05?,but lower than that of HC group?P<0.05?,but no statistical significance compared with the IG,CG group?P>0.05?.The expression level of ER?and GPR30 protein in group AG was significantly lower than that in group IG and CG?P<0.05,respectively?,but there was no significant difference between IG and CG group?P>0.05?.Conclusion:The expression of E2,ER?,ER?,GPR30 in peripheral blood of gout patients were significantly decreased,suggesting that the estrogen and its receptor may be involved in the pathogenesis of gouty arthritis.
