ObjectiveIn recent years, with the changes in people's lifestyle and dietary structure, the incidences of coronary heart disease are increasing in China.Although coronary angiography can intuitively determine the degree of coronary artery stenosis,it is still an invasive procedure. In this study,we aim to explore the levels of plasma Thrombospondin-1(TSP-1)and Thrombospondin-2(TSP-2)in patients with different degrees of coronary artery stenosis. At the same time,we also want to further explore the relationships betweenTSP-1?TSP-2?Gensini score(GS)and inflammatory cytokines(MMP-9 and hs-CRP).This study provides a quick and convenient clinical basis for the early and active intervention in the occurrence and development of coronary heart disease, and also provides an important intervention, treatment and research target for coronary artery stenosis.MethodsBlood plasma samples were obtained from 106 patients who were admitted to the Department of Cardiology,Subei People's Hospital of Jiangsu Province,China, from July 2017 to December 2017.All the patients were treated with percutaneous coronary intervention and blood examination.Among them,79 cases diagnosed with coronary artery stenosis were treated as the experimental group, and 27 cases with normal coronary angiography were served as the control group. All patients met the inclusion criteria and did not meet the exclusion criteria. The experimental group was regrouped according to the Gensini scores,27 cases in mild stenosis group(Low score group,Gensini score?12 points),26 cases in moderate stenosis group(Medium score group,Gensini score= 12?60 points),26 cases in severe stenosis group(High score group,Gensini score?60 points).Basic clinical data such as gender, age, history of diabetes, laboratory parameters such as fasting blood glucose, plasma total cholesterol(TC),triglyceride(TG),etc.and coronary angiography results were recorded for all subjects.Enzyme linked immunosorbent assays were used to detect the concentration of TSP-1,TSP-2,MMP-9,and hs-CRP in fasting plasma of the subjects in each group.EXCEL?PRISM and SPSS23.0 software were used for statistical analysis.Results1.There were significant differences between three groups in TC?creatinine?neutrophils?smoking rate and the prevalence of diabetes(P<0.05).Left ventricular ejection fraction(LVEF)decreases as the score increases. The traditionally protective values of HDL-C and ApoA1 decreased with increasing Gensini scores, which was also statistically significant(P<0.05).There were no significant differences in age,BMI,blood pressure,triglycerides(TG),low-density lipoprotein(LDL-C)and the mean platelet volume(P>0.05).2.With the increase of Gensini score, the plasma TSP-1 and TSP-2 levels showed an upward trend.Plasma TSP-1 level was not statistically different between the Gensini low score group and the Gensini medium score group(P>0.05)and there were statistical differences between all the other groups(P<0.05).There was no difference in plasma TSP-2 between adjacent two groups(P>0.05).Control group and the Gensini medium score group began to show statistical differences(P<0.05)while the control group and the Gensini high score group were statistically significant(P<0.01).Linear correlation analysis showed a significant correlation between plasma TSP-1(r=0.467>0,p<0.01),TSP-2(r=0.5134>0,p<0.01)and Gensini scores.3.Analysis between TSP-1,TSP-2 and coronary artery stenosis-related inflammatory cytokines MMP-9?hs-CRP showed that there was a significant positive correlation between TSP-1 and MMP-9(P<0.001,r=0.3940)and hs-CRP(P<0.001,r=0.4979)(P<0.01).There was also a correlation between TSP-2 and MMP-9(P=0.0138,r=0.2385),hs-CRP(P=0.0357,r=0.2042)(P<0.05),but lower than TSP-1.4.With TSP-1 as the dependent variable,Gensini score and hs-CRP were independent predictors of plasma TSP-1(P<0.05);With TSP-2 as the dependent variable,Gensini score and LVEF are independent predictors of plasma TSP-2(P<0.05).Logistic regression analysis showed that TSP-1,age,and hs-CRP were independent risk factors for coronary artery stenosis, and HDL-C was an independent protective factor(P<0.01).5.The Receiver Operator Characteristic Curve(ROC)indicates that the areas under the TSP-1,TSP-2,and hs-CRP curves are 0.812,0.742,and 0.845,respectively,which are all greater than 0.5,P=0.00<0.01.The results show that the authenticity of TSP-1 diagnosis is close to the current clinical indicator hs-CRP,while the authenticity of TSP-2 diagnosis is lower than that of TSP-1 and hs-CRP.Conclusions1.There is a significant positive correlation between plasma TSP-1?TSP-2 levels and the degree of coronary artery stenosis.2.There is also a positive correlation between plasma TSP-1?TSP-2 levels and plasma inflammatory cytokines.3.Plasma TSP-1 and TSP-2 levels are of great value in the evaluation of preoperative coronary artery in patients with coronary artery stenosis.TSP-1 was superior to TSP-2 on the predictive ability of coronary stenosis.4.TSP-1 and TSP-2 are an important intervention, treatment and research target for coronary atherosclerosis...
