Objective:The aim of study was to evaluate the diagnostic value of the improved elastic scoring,elastic strain rate ratio(SR),virtual touch tissue quantification(VTQ)in the identification of benign and malignant breast tumors.Methods:A total of 62 patients who had been hospitalized for breast focal lesions in breast surgery were enrolled.Routine ultrasonography,compression ultrasonography and acoustic radiation force impulse imaging(ARFI)were performed.Among them,routine ultrasonography was used as a two-dimensional positioning basis,compression ultrasonography is divided into improved elastic scoring and elastic strain ratio and VTQ method is used for acoustic radiation force impulse imaging.First,the elastic strain rate ratio(SR)and VTQ velocity value were used to draw the receiver operating characteristic curve(ROC),the area under the sampling line and the optimal threshold value of elastic strain rate ratio(SR)and VTQ velocity value was calculated.At the same time,at the optimal threshold,the sensitivity,specificity and accuracy of these two and improved elastic scoring in the identification of breast benign and malignant lesions were compared.Results:The postoperative pathological results of the breast lesions were gold standard.(1)The elastic score of the malignant tumor was 4.118±0.769,and the elastic score of the benign tumor was 2.536±0.793.The sensitivity,specificity and accuracy of the improved elastic scoring were 76.5%,89.3% and 82.3% for the differential diagnosis of benign and malignant breast lesions.(2)The SR value of the malignant tumor was 4.094±0.810,and the SR value of the benign tumor was 3.233±0.574.The area under the ROC sampling line of SR was 0.818.The optimal threshold value of SR for differential diagnosis of benign and malignant breast lesions was 3.775.The sensitivity,specificity and accuracy of SR were 67.6%,89.3% and 77.4%.(3)The VTQ value of the malignant tumor was 4.151±1.034m/s,and the VTQ value of the benign tumor was 3.198±0.498m/s.The area under the ROC sampling line of VTQ was 0.745.The optimal threshold value of VTQ for differential diagnosis of benign and malignant breast lesions was 3.825.The sensitivity,specificity and accuracy of VTQ were 58.8%,96.4% and 75.8%.(4)The elastic score,SR value,VTQ value of malignant tumors range from 3 to 5,2.13 to 6.53,1.50 to 6.13m/s.Conclusion:(1)The difference of improved elastic scoring between benign and malignant breast lesions was statistically significant,and benign lesions were mostly in grade I ~ III,and malignant lesions were mostly in grade ?~?.(2)The difference of SR and VTQ between benign and malignant lasions was statistically significant.(3)The sensitivity,specificity and accuracy of the three diagnosis of breast benign and malignant lasions have no statistically significant difference.(4)There is no essential difference between the three in the diagnosis of benign and malignant breast tumors.(5)The elastic span of the malignant tumors is relatively large.
