Background:Poor quality of adult sleep is mainly caused by sleep disruption,long-term sleep disturbance is closely related to cardiovascular diseases such as hypertension and ischemic heart disease.Clinical electrocardiogram can only capture abnormal electrophysiological signals during acute episodes of cardiovascular disease,some mild cardiovascular abnormalities,especially it is a simple and easy monitoring method for nighttime attacks.PM2.5 may lead to premature death from cardiovascular disease,and sleep quality assessment may provide early warning indicators for the premature death of cardiovascular disease against PM2.5.OBJECTIVE:A panel study was conducted to assess the effects of PM2.5 on adult sleep quality and to explore early warning indicators of the effects of PM2.5 on premature death from cardiovascular disease.METHODS:Taking the adult of 18 years old and above in 50 representative households in ShibaLidian Township of Beijing as the research object,the sleep quality was evaluated by PSQI scale.The variance analysis was used to compare the sleep quality of PM2.5 pollution in winter and summer to cardiovascular patients and the general population impact difference;PM2.5 exposure level was evaluated by Beijing daily average PM2.5 concentration,and Spearman correlation was used to analyze the correlation between PM2.5 log concentration and adult sleep quality score and 7 item scores.Based on the total quality of sleep quality,a two-level distributed lag nonlinear model(DLNM)was constructed to evaluate the effect of PM2.5 on PSQI sleep quality scores.RESULTS:In this study,the Spearman method was used to analyze the correlation between sleep quality and PM2.5 of 50 households in Beijing Shilidian Township.Based on the two-level DLNM model,the lagging effect of PM2.5 on sleep quality was discussed.The main findings are as follows:i.The general characteristics of the respondents:133 adults over the age of 18 participated in the survey in 50 households,and 133 respondents(male/female)included 65 people with sleep-disrupted sensitivity(48.87%),including 46 with high blood pressure(34.59).%),14 people with diabetes(10.53%),5 patients with coronary heart disease(3.76%),6 people with asthma(4.51%),and 36 people with other adults(27.07%);the average age of the subjects was 45.85±14.05 years old,culture The degree is 28(21.05%)with university degree or above,94(70.68%)in-service,and 39(29.32%)retirees.ii.Results of sleep quality assessment:133 respondents reported that the quality of sleep was good,better and worse,21(15.79%),88(66.17%)and 22(16.54%).iii.Beijing winter adult sleep problem composition and sleep quality daily questionnaire score results:winter adult daily sleep quality seven evaluation items in the frequency of the proportion of high to low order is sleep efficiency,daytime abnormalities,subjective sleep quality,sleep disturbance,sleep duration,delayed sleep and drug sleep,133 adult PSQI total score(6.44±2.19)points,cardiovascular patients PSQI total score(6.87± 2.35)points,other sleep-disrupted people PSQI total score(6.21±2.04)points,the general population PSQI total score(6.20±2.05)points,the total score of PSQI in winter cardiovascular patients was significantly higher than the general population by analysis of variance(p<0.05).iv.Beijing summer adult sleep problem composition and sleep quality daily questionnaire score results:summer adult daily sleep quality seven evaluation items in the frequency of the proportion of high to low,followed by daytime abnormalities,subjective sleep quality,sleep efficiency,sleep disturbance,sleep duration,delayed sleep and drug sleep,116 adult PSQI total score(5.75 ± 1.82),cardiovascular patients PSQI total score(6.03±1.94),other sleep-disrupted people PSQI total score(6.10±1.95)points,the general population PSQI total score(5.45±1.64)points,the variance analysis of the total score of PSQI in summer cardiovascular patients was significantly higher than the general population(p<0.05).v.The average daily variation of PM2.5 during the survey period:during the winter survey,the atmospheric PM2.5 concentration in Beijing averaged 135±105?g/m3.the lowest was 24?g/m3.the highest was 381?g/m3,and the exceeded days were 17 days(60.71%);During the summer survey,the atmospheric PM2.5 concentration in Beijing averaged 37±19?g/m3,the lowest concentration was 9?g/m3,the highest value was 89?g/m3,and the exceeded standard(PM2.5 daily average concentration>75?g/m3)was 1 day(3.57%).vi.Correlation between sleep quality score and PM2.5 in the survey:This study used Spearman correlation to analyze the total sleep quality score of Shibalidian Township,7 sleep quality special scores and logarithm conversion during the survey period PM2 5 and Correlation of lag 1-7day PM2 5 concentration,the results showed that PM2.5 and residents'total sleep quality score(p=0.008).,subjective sleep quality score(p=0.037),sleep duration score(p=0.043),sleep efficiency Scores(p<0.001)and sleep disturbance scores(p = 0.027)were significantly correlated;in addition,the sleep efficiency score was also significantly negatively correlated with PM2.5(Lag2)with 2 days of lag(p = 0.003);PM2.5 was significantly associated with sleep efficiency scores(p<0.001)in sleep disturbance-sensitive and CVD patients.There was no significantly correlated between sleep quality scores and 6 sleep quality scores and PM2.5(LagO-Lag7)(p>0.05).vii.Impact of PM2 5 on sleep quality:The contribution of individual differences to the total score of sleep quality was 52.3%(ICC = 0.523).The effect of PM2.5 on sleep quality was analyzed by two-level DLNM.The fitting results suggest that PM2.5 in Shibalidian has a significant cumulative lag effect on the total score of sleep quality of cardiovascular patients.The cumulative lag effect of PM2.5 persists.For 5 days,for every increase 10 ug/m3 in PM2.5,the total score of sleep quality of cardiovascular patients increased by 0.016(95%Cl:0.003-0.029)points;the results of two-level DLNM fitting showed that PM2.5 had a total sleep quality for the general population.There was no significant hysteresis(p>0.05),and no significant cumulative hysteresis(p>0.05).Conclusion:PM2.5 mainly reduced sleep quality by affecting sleep duration,sleep efficiency and sleep disturbance.The total score of PSQI in cardiovascular patients was significantly higher than that in the general population.PM2.5 had significant effects on the total score of sleep quality of cardiovascular patients.Cumulative hysteresis,sleep quality may be an early indicator of the effect of early warning of PM2.5on cardiovascular disease.The results of this study suggest that sleep disorders may be an early warning signal for cardiovascular disease.Cardiovascular patients should go to the hospital in time for sleep disorders and timely health protection.
