Study On Wage Threshold Effect On Payment Of Basic Medical Insurance Fund For Urban Employees

Basic medical insurance system for urban employees is the earliest established one in the socialization of China's medical insurance system,which has been running for nearly 20 years.Any reform concerning the system would cause heated discussions,causing much social attention.The medical insurance fund,which is the material basis for the operation of the system,will directly affect the benefit of the insured.However,the policy that ‘Base payment on financing,balance between financing and payment,get a slight balance' means that payment should be adapted to financing,the existing medical and health services and so on.It's worth attention that the payment of the medical insurance fund is a kind of economic activity,so the payment will be affected by the general economic climate.With the deepening of the reform of the economic system and the continuous development of economy,China's medical and health services have developed a lot with rapid increase in medical and health expenditure and payment of medical insurance fund.Meantime,the wage of employees has also been affected by the economic development.The wage of employees not only constitutes one of sources of medical insurance fund,but also has an impact on medical consumption behavior and then the payment of medical insurance fund.So this index is quite important and relevant to the research.This paper will establish the wage-threshold effect model based on the economic development to study on the relationship among the three indexes,and offer advice about reform of basic medical insurance system for urban employees according to the result of the research.Firstly,the paper will summarize related studies from other scholars and define related concepts,and then comb the history of the system and briefly evaluate the design of the system.The current situation of the payment of the fund will take on the form of statistical analysis from the angle of the changes in budget and balance,region and economic effect.The result shows that the financing of the fund is generally greater than the payment,but in recent years there exists balance pressure.There are regional differences in the payment of the fund.The payment of the fund appears obvious economic effect.Secondly,this paper will describe and analyze the current situation of China's economic development from three aspects of GDP,per capita GDP and changes in Theil index,and a conclusion is drawn that the level of economic development in China isincreasing and regional disparity in China is relatively narrowing.Then the paper will analyze the impact of economic development on wage.As the next step,the paper will describe the current situation of wage from the angle of urban employees' total wages and average annual wages as well as the wage income in per capita disposable income,and the conclusion is that the wage level of China is rising and there's a decline in the proportion of disposable income.In the same way,the paper will analyze the effect of wage on the payment of medical insurance fund.The relationship between the three variables of funds-payment,level of economic development and wage level,will provide the premise for empirical research.Next,this paper will use the provincial panel data for empirical research,establishing fixed effect models,and establish threshold effect models of total and personal angle respectively on the basis of the results of the fixed effect models,by which the coefficient on economic development of the fixed effect models and threshold effect models can be compared.Results of thresholds are given with positive effect,and the coefficient is increasing between intervals.The coefficient of the total-model is always lower than the one of fixed effect model,and the coefficient of per capita-model finally exceeds the one of fixed effect model in the third interval.The total-model will provide a reference for the balance of funds in the macro level,and the per capita-model will provide reference for the improvement of the system from the micro perspective.At last,this paper will offer advice on the reform and optimization of the basic medical insurance system for urban employees,based on the results of the analysis of the statistics and the empirical analysis in order to better protect the insured's health rights,improve health and promote recovery and reproduction of human resources and social stability.
