The new curriculum reform put forward a new evaluation concept,in addition to pay attention to students' learning results,the evaluation should pay more attention to the students' mathematical thinking,problem solving,and evaluation of the learning process,which focus on the emotional attitude of students' performance in math activities,and lay emphasis on the diversification of evaluation subjects and evaluation methods."Green index evaluation" which is implemented in Shanghai turn the test evaluation model into a comprehensive evaluation.Thus,how to evaluate students' learning process and result comprehensively and objectively shows appraisal development function will become the trend of the development of education evaluation.Formative assessment abandons the traditional evaluation model which is oriented by results,and it pays more attention to the evaluation of the students' learning process,emphasizes the feedback of students' learning situation.Consequently,it promotes the development of students,which in line with the current concept of evaluation.So it is worth advocating and practicing.This paper expounds the connotation,theoretical basis and characteristics of formative evaluation by compiling the literature of formative evaluation,and combs the main research achievements and the latest development trends of formation evaluation at home and abroad,and forms the research summary,and points out the direction of this research.According to the characteristics of formative assessment and the steps of learning evaluation,the implementation steps of formative assessment are discussed in detail.On the basis of theoretical research,through the cooperation with primary school mathematics teachers,carry out the formation of the evaluation of practical research.First,build the evaluation objectives,and then select the evaluation method to carry out the evaluation.Finally,analysis the evaluation information,and the results are fed back to the students and teachers in order to promote the achievement of goals which are not achieved.Through the reflection of the practice process,the author summed up the role of formative evaluation,put forward recommendations for carrying out formative assessment effectively,provide guidance and practical basis for the further development of primary school mathematics formative assessment.This paper divided into five chapters: The first chapter introduces the research origin,the research significance,the research content,the research method and the research thought,and summarizes the relevant literature at home and abroad.The second chapter summarizes the formation evaluation,including the connotation,theoretical basis and characteristics of formative evaluation.In the third chapter,based on the general steps of learning evaluation,combining with the characteristics of formative assessment,the steps of formative evaluation are described in detail.The fourth chapter discusses the concrete steps of formative assessment.The fourth chapter designs the evaluation of the 3 cases according to the formative evaluation steps,and the practice is carried out in the practice school.The analysis feedback from the construction of the evaluation goal to the final evaluation result is presented,and the formation evaluation template is provided for the first-line teachers.The fifth chapter summarizes the advantages of formative evaluation in practice research,and puts forward some suggestions according to the deficiency of formative assessment.
