ObjectiveThis clinical study surveys cognitive function in elderly hypertension,analysis the impact of hypertension on the cognitive domain,the influence factors of high blood pressure lead to cognitive impairment,and TCM syndrome characteristics,further explores the pathological mechanism of hypertension leading to cognitive dysfunction.In order to providing certain guiding significance to clinical prevention and treatment of hypertension with MCI.MethodsA cross-sectional survey was conducted to select 87 patients with high blood pressure,.According to Petersen MCI screening criteria,which were divided into MCI group(n=20)and NC group(n=67 cases).Proceeding Neuropsychological tests,inclunding the application state of simple intelligent scale(MMSE),Montreal cognitive assessment Beijing edition(MOCA)to assess the overall cognitive capacity,and test the memory(N5,N1N5,complex graphics delay),language(verbal fluency,Boston named),space force(picture clock,complex graphics imitation),concentration(digit symbol,attachment test A)and execution(color word test time C,attachment B)such as cognitive domain.Collection of gender,age,education level,marital status,occupation(or profession before retirement time),merge disease memory decline,systolic blood pressure(SBP),diastolic blood pressure(DBP),medical history and other general conditions and cholesterol,triglyceride,low density lipoprotein cholesterol(ldl-c),high-density lipoprotein cholesterol(hdl-c),homocysteine(HCY),liver and kidney function,fasting blood glucose,glycosylated hemoglobin and other clinical data.Functional magnetic examination of the head function.Application of four diagnostic information scales of Chinese medicine for TCM syndrome investigation.Results1 In this study,the age of hypertension MCI group was higher than that in the control group(P<0.05),and the MCI group was lower than the control group(P<0.001)5 and the difference was statistically significant.The systolic and pulse pressure of MCI group was higher than that of NC group,and the difference was statistically significant(P<0.05).2 In this study,MMSE and complex graph imitation scores of the MCI group were lower than that in the NC group,and the connection test A and B were longer than the NC group,and the difference was statistically significant(P<0.05).3 This study hypertensive cerebral white matter lesions in the age of high group less cerebral white matter lesions(P<0.05),the incidence of cerebral white matter lesions MCI group was 43.9%,significantly higher than that of without cerebral white matter lesions group,the difference was statistically significant(P<0.001);N5,N1N5,speech fluency,Boston naming and digital symbol score were lower than those in the white matter lesion group,and the difference was statistically significant(P<0.05).4 This study shows that syndrome of deficiency of kidney essence of patients with hypertension and MCI is the most common,accounting for 35%,followed by the syndrome phlegm-dampness obstruction,deficiency of both qi and blood,upper hyperactivity of liver fire,and blood stasis blocking brain.ConclusionIn addition to human factors such as age,level of education,higher systolic blood pressure and pulse pressure and the resulting in a decline in cognitive functions and should be to maintain the flow of blood to the brain and ensure adequate cerebral perfusion,individualized slow decompression,and resistance to atherosclerosis therapy may prevent cognitive decline in patients with high blood pressure.The overall cognitive function,spatial force,attention and executive force of the elderly patients with hypertension decreased significantly,and there was a decline in memory.Hypertension patients with cerebral white matter lesions with delay memory,verbal memory and concentration,cognitive dysfunction rate increased significantly,to control the risk factors for cerebral white matter lesions may be able to prevent cognitive decline in patients with high blood pressure.The basic pathogenesis of hypertension combined with MCI is the deficiency of kidney essence,and the basic pathological factors are phlegm,liver fire and blood stasis.
