| [Objective]Investigation and statistical analysis of 285 schizophrenia inpatients to observe the distribution of TCM syndromes in schizophrenia.At the same time,a preliminary discussion was made on the correlation between TCM syndrome types and glycolipid metabolic index,which provides ideas for the standardize construction of TCM and the TCM treatment of schizophrenia complicated with glycolipid metabolic disorder.[Methods]In this survey,patients with schizophrenia were studied,and the schizophrenic TCM Syndrome Observation Rating Scale was formulated with this topic.The epidemiological survey data,TCM fourth round diagnostic information,and laboratory inspection index from 285 inpatients from Peking Changping District Chinese and Western Medicine Hospital,Capital Medical University Affiliated Anding Hospital,Peking Dongcheng District Mental Health Hospital,and Peking University Sixth Hospital from May 2017 to March 2018 were collected for statistical analysis to observe the distribution of TCM syndromes in schizophrenia and to explore the correlation between syndrome and glycolipid metabolic.[Results]1.The epidemiological survey condition:The ratio of males and females in the 285 schizophrenia patients is 1.31:1.The ages of the patients were mainly distributed in 31-40 and 41-50 age groups.Demographics show that unmarried(68.07%),junior and above high school education(81.75%),smoking(51.58%),overweight and obesity(23.15%)are the characteristics;work stress,emotional events(52.28%)are the main predisposing factors;olanzapine(36.84%),risperidone(16.49%),quetiapine(25.96%)and other second-generation antipsychotic drugs are the main treatment drugs;long duration,high recurrence rate,repeated hospitalization and others are disease features.2.Glycolipid metabolic index condition:19.84%of patients’ FBG above upper limit,a large number of patients’ HbAlc index untested or missing;28.17%of patients’ TC above the upper limit;34.92%of patients’ TG above the upper limit;15.48%of patients,HDL-C below the lower limit;23.02%of patients’ LDL-C above the upper limit.The dyslipidemia is mainly caused by the elevation of TG,TC,and LDL-C,and the proportion of the HDL-C reduction is relatively small.3.TCM syndrome distribution condition:The most common five TCM syndromes in 285 schizophrenia patients are hepatic depression(21.30%),spleen deficiency(17.19%),intense liver firepattern(15.43%),heart deficiency(13.33%),and phlegm dampness(11.93%).Some of the patients were "undocumented"(24.21%).Patients with single syndrome accounted for 51.93%,21.40%for patients with two syndromes,and 3.86%for three syndromes,wherein hepatic depression,spleen deficiency,and phlegm dampness are the most common syndromes in compound syndromes.4.Correlation between TCM syndrome type and glycolipid metabolic index:the TG level in the phlegm dampness group and heart deficiency group are similar(P>5)and high,and there was a significant difference compared with other groups(p<0.05);There was no significant difference of TC level between each syndrome group(P>0.05);the levels of LDL-C in phlegm dampness,heart deficiency and undocumented group are statistically different from those in other group(p<0.05);the level of BMI in phlegm dampness and spleen deficiency group are statistically different from those in other group(p<0.05);the FBG(Fasting blood glucose)level in spleen deficiency group is high and is statistically different from those in other group(p<0.05)[Conclusion]1.The symptoms of schizophrenia with TCM are hepatic depression,wherein hepatic depression,spleen deficiency,and phlegm dampness are the most common syndromes in compound syndromes.The symptoms of the disease reflect the etiology,pathogenesis,and deficiency-excess mixing of hepatic depression and yin-yang disorders.Some of the patients are "undocumented",which show the complexity and uncertainty of the distribution of TCM syndromes in schizophrenia.2.There are correlations between the body mass index,fasting blood glucose,triglyceride,low-density lipoproteins in serum lipid index and symptoms of schizophrenia with TCM.3.The psychiatrist needs to pay more attention to the risk of abnormal glucose metabolism in schizophrenia patients,and to actively monitor the glycosylated hemoglobin,postprandial blood glucose and other indicators in order to find and correct abnormal glucose metabolism early. |
