Based On FMRI, The Effect Of Yishen Zhuyu Method On The Resting Brain Network Of Tonic And Less-active PD Was Studied

Objectives(1)Tostudy the clinical efficacyof Yishen Zhuyu method in treating akinetic-rigid PD;(2)To study the changes of brain function network after treatment with Yishen Zhuyu method;(3)To study the correlation between the changes of brain function network and scale after treatment of Yishen Zhuyu method.MethodsThis study included 9 patients with akinetic-rigid PD and 9 healthy controls matched with their age,sex and education.The experimental group was treated with Pei Yuan Jie Jing Tang and the rest functional magnetic resonance scan was performed before and after treatment respectively,and the control group had only one resting state functional magnetic resonance scan as a control.Patients in the treatment group were scored with Hoehn-Yahr classification(HY classification),Unified Parkinson’s disease rating scale exercise examination(UPDRS Ⅲ),Parkinson’s disease non-motor symptom rating scale(NMSS)and TCM syndrome quantitative scale before and after treatment.Six brain networks were found using independent component analysis:SMN,aDMN,pDMN,LFPN,RFPN.The Granger causal analysis model was used to analyze the causal effects between the patient group and the control group before and after the treatment.The correlation between the scores of the four scales before treatment and the effect of brain network causality was analyzed,and the correlation between the difference of the scores of the scales and the changes of the brain network effects after treatment was also analyzed.Results(1)Based on the Granger causality analysis model,it is concluded that the causal connection between brain networks of Parkinson’s patients before the treatment of Yishen Zhuyu method is that the changes of SMN affect the RFPN.while after thetreatment of Yishen Zhuyu method,the changes of aDMN and SN affect SMN.(2)To observation before and after treatment based on Granger causality Analysis Model,The correlation between the changes of the above six brain networks in HY grade,TCM syndrome scale(TCM),uniform Parkinson’s disease scale(PD)-motor examination,Parkinson’s disease non-motor symptom scale(NMSS),The results are as follows:Before treatment,with the increase of the score of UPDRSIII,the change of the DMN affected the change of FPN,and the two factors were positively correlated.with the increase of the score of HY grade,the change of pDMN caused the change of LFPN,and the correlation between them was positive correlation.with the increase of the score of TCM grade,the changes of FPN affected the changes of SMN.There was a positive correlation between them.After treatment,there was no correlation between changes in the uniform Parkinson’s Rating Scale-motor check score and the Granger causality between brain networks.and there was a positive correlation between the changes of the scores of TCM syndrome scale and the brain network before and after treatment.The results showed that the change of SN influenced the change of SMN,and there was a positive correlation between the change of the score of non-motor symptom scale and the brain network before and after treatment.The change of SN affects the change of SMN.Conclution(1)Compared with the normal people,the dynamic changes in the brain network of theakinetic-rigid PD patients showed a significant change.The SMN affected the RFPN,suggesting that the patient’s cognitive dysfunction was related to sensorimotor dysfunction.(2)After treatment with the Yishen Zhuyu method,the brain network of the akinetic-rigid PD patients changed,becoming the aDMN and SN affect the SMN.Considering the Yishen Zhuyu method may improve motor symptoms through the integration of sensory information.(3)There was a correlation between the change of scale score and the causality effect between brain network,and the causal effect between the network before and after treatment was mainly around SMN,SN and DMN,which confirmed that the central mechanism ofYishen Zhuyu methodin treatment ofakinetic-rigid PD may be to improve the integration of cognition and sensory afferent,and then alleviate the rehabilitation disorder of motor symptoms.
