Objective:To investigate the influencing factors of Gastroesophageal reflux disease(GERD)and to explore the relationships between asthenia and sthenia syndrome groups based on the high resolution esophageal measurement technique(HRM).Method:In this study,patients come from Gastroenterology clinic in Beijing Traditional Chinese Medicine Hospital.According to the results of endoscopy and/or 24 hours esophageal PH monitoring,123 GERD patients were included in our study who met the diagnostic criteria and inclusion criteria,exclusion criteria after screening of the selected cases.To collect and summarize the clinical symptoms,patients were divided into sthenia syndrome group and asthenia syndrome group.Then they took the high resolution esophageal pressure measurement while recording the dynamic parameters of esophageal sphincter and esophageal body.The data were analyzed by statistical method.Results:1 General Information:This paper collected a total of 123 cases of GERD patients,male 62 cases(50.4%)and 61 females(49.6%).Ages ranged from 15 years to 83 years,with an average of 47.12 ± 14.18.Patients was mainly in young and middle aged-based.BMI ranged from 18.2-28.8,with an average of 23.6±2.5.The course of the disease was mainly concentrated from 3 months to 5 years.2 GERD of TCM syndrome distribution:123 cases of GERD patients collected in this article,The syndromes could be categorized into six pattems,stagnated Heat in Liver and Stomach Syndrome 46 cases(37.4%),Dan-Heat invading Stomach type in 20(16.3%),blood stasis in collaterals 4 cases(3.3%),Qi phlegm 12 cases(9.7%),Imaginary inverse gas card 23 cases(18.7%),syndrome of dampness-heat due to spleen deficiency 18 cases(14.6%).Furthermore,patients were divided into two groups,sthenia syndrome group 70 cases and asthenia syndrome group 53 cases.3 Sexual distinction,age groups,body mass index and course of disease were not statistically significant,according to sthenia syndrome and asthenia syndrome of GERD patients(P>0.05).4 Functional comparison of UES and LES:In resting pressure of LES,asthenia syndrome group was higher than sthenia syndrome group(P<0.05).In residual pressure of UES,resting pressure of LES?residual pressure of LES,LES length,sthenia syndrome and asthenia syndrome groups were not statistically different(P>0.05).5 Dynamic parameters comparison of Esophageal body:In distant contraction index(DCI),7,3 cm above the LES in amplitude and 7,3 cm above the LES-wave duration,asthenia syndrome group was lower than sthenia syndrome group(P<0.05).In swallowing speed,the percentage of ineffective swallowing and the percentage of large peristalsis interruption,sthenia syndrome and asthenia syndrome groups were not statistically different(P>0.05).Conclusion:Sthenia syndrome and asthenia syndrome groups from GERD patients are associated with resting pressure of LES,7,3 cm above the LES in amplitude,7,3 cm above the LES-wave duration and distant contraction index.The expression of esophageal motility insufficiency in the asthenia syndrome group.
