Objective:This article explore the combined obstruction of the left colon cancer and rectal cancer surgical methods, compare with intraoperative colonic lavage anastomosis of the surgical approach and conventional issue of proximal colostomy, the second phase of tumor resection of proximal fistula in the correlation between the statistical data,to prove the feasibility and security of a resection and anastomosis of the joint in colonic lavage way of surgical for colorectal cancer in the treatment of left colon cancer and rectal cancer Methods:This study retrospectively analyzed the clinical data of 70 patients with left colon cancer and rectal cancer who underwent surgical treatment from January 2013 to December 2016 in Pingyi County People's Hospital of Shandong Province.Thirty-five patients of the tratment of intraoperative colonic lavage anastomosis were selected as the experimental group, and another 35 patients were treated with issue of proximal colostomy and the second phase of tumor resection of proximal fistula as control group. The data of the two groups were statistically analyzed, and the general situation of the two cases were compared to confirm whether the two groups were comparable.Comparison of the two cases of surgical data, including the operation time, the amount of bleeding in surgery;the postoperative recovery of the two groups were compared with the operation data of the two groups of cases, including the incidence of postoperative anastomotic fistula and the rate of surgical infection(including surgical incision and intraperitoneal infection rate);the cost and length of stay of hospitalization and were compared between the two groups;the quality of life scores were compared between the two groups. Results:There were 9 cases in selected total 70 cases of complications, including incision infection in 4 cases, which were cured by the local drainage, dressing, physical therapy;and anastomotic leakage occurred in 2 cases, which were cured by fasting, smooth drainage, parenteral nutrition support recovery;there are 3 cases of abdominal infection, which were cured by the peritoneal rinse drainage, sensitive antibiotics after application. All nine patients were clinically cured. Two groups of patients in the postoperative 14-50 days were clinically cured. There was no significant difference in the incidence of abdominal infection, the rate of surgical incision infection and the rate of anastomotic fistula(P> 0.05)in the general data of the two groups(including age,sex ratio, and medical basic diseases). There were significant differences in the operation time, the amount of surgical bleeding, the hospitalization time and the hospitalization cost compared with the control group(P <0.05);the operation time and the amount of surgical bleeding in the test group were lower than those in the control group;the hospitalization and hospitalization expenses were also lower than those in the control group;the scores of the experimental group were significantly higher than those of the control group(P <0.05);the scores of the social function, psychological function, physical function and material life of the experimental group were significantly higher than those of the control group. Conclusion:It is safe and effective to treat left colorectal cancer with acute intestinal obstruction by intraoperative colon lavage of resection and anastomosis. The surgical approach not only avoid the patient for secondary surgery, but also make patients recover fast, short hospital stay, short hospital costs, high quality of life,in conditions permitting,it is worth in the future clinical treatment to promote.
