OBJECTIVE:Coronary heart disease(CHD)is an Ischaemic Heart Disease caused by thickening of the coronary wall and elastic damage.CHD is a multifactorial disease,in which inflammatory factors play very important roles.Therefore,there is a view that CHD is a chronic inflammatory disease.For chronic inflammation,cellular immunity often plays an important role in the pathogenesis.While the thymus is an important immune organ involved in cellular immunity,there are lots of research proved that the function of adults' thymus still persistence.During the last decades,some studies have focused on the relationship between circulating Th cells and the incidence and severity of CHD.However,there is no evident shows the correlation between Th cells in the thymus and CHD.In this study,we compared the characteristics of T cells in blood and thymus tissue between CHD patients and non-CHD patients,and discussed the role of thymus in CHD.METHODS:Sixty adult patients underwent thoracotomy for the first time from March 1,2017 to May 1,2017,including 45 patients with CHD and 15 patients without CHD.Specimens came from central venous blood and thymus tissue,which were taken early in the operations.Naive T cells and Thl cells were labeled with CD45RA and CCR5,respectively.Blood flow cytometry and immunohistochemical of thymus paraffin sections were used to identify the results.Clinical data collected from preoperative routine examinations and tests.Imaging and pathology data was provided by experienced specialists.The patients in the two groups were ordered according to the date of operation.The resulting data was processed by SPSS 23.0(IBM)software.RESULTS:45 cases in the case group and 15 cases in the control group.There was no significant difference of TI between the two groups.In blood flow cytometry,MFI of CCR5 shows significant difference in two groups(165.44 ± 25.50,121.79 ±77.95(p=0.02)),and it has correlation with SYNTAX score in patients group when analyzed by liner regression(R2=0.799).MFI of CD45RA(573.36±165.60 and 625.08±136.60,respectively(p=0.234)),percentage of CCR5(12.13 ± 5.85%and 10.56 ±3.00%,respectively(p=0.182))and percentage of CD45RA(29.05 ± 9.85%and 35.38 ±13.06%,respectively(p=0.092))do not show significant difference.In immunohistochemical of thymus,the number of CD45RA+ cell aggregates shows significant differences in two groups,the mean number is 3.93 ± 2.87 and 5.60 ±2.23,respectively,with p value=0.045.But it doesn't show a compacted association with SYNTAX score in CHD patents,with R2=-0.145.The number of CCR5+ cell aggregates does not show significant difference(1.533 ± 1.77 and 1.200 ± 1.24,respectively(p=0.478)),while the size of aggregates marked by either antibody does not show significant difference.CONCLUSION:The excessive differentiation of naive T cells into Thl cells in thymus may take part in CHD pathology.Increased Thl cells which come from thymus in the blood may play important role in pathology of CHD.
