Part ? Study on the relationship between serum 25(OH)D and hypertension[Objective]In this study,we detected and compared the serum levels of 25-hydroxy vitamin D in normal subjects and patients with hypertension,so as to explore the correlation between vitamin D and hypertension and its influencing factors.[Methods]1.Research object:Subjects from March 2016 to January 2017 to the Jiangsu Province People's Hospital Medical Center for health examination population,age range 45 to 65 years old,through the inclusion and exclusion criteria,included 86 patients with hypertension,which is also included in the hypertension group,age and gender matched 88 healthy people as control group.2.Collecting clinical data:Fasting blood test to fasting blood glucose(FBG),total cholesterol(TC),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),triglyceride(TG),uric acid(UA),creatinine(Cr)and other biochemical indicators.Serum 25-hydroxy vitamin D[25(OH)D]levels were determined by enzyme-linked immunosorbent assay.From the hospital medical center physician measured height,weight,waist circumference and other anthropometric indices and systolic blood pressure(SBP),diastolic blood pressure(DBP),and calculate body mass index(BMI)and pulse pressure(PP).Detailed inquiry,physical examination of the patient's past illness and medication history and other information.3.Statistical software analysis:Using SPSS 19 statistical software to analyze data.Continuous variables are represented by mean±standard errors.The categorical variables are expressed in terms of numbers or percentages,and logarithms of non normal distributions are transformed by logarithms.The parameters of two groups were compared with two groups of independent sample T test;count data were compared by chi square test;parameters between Pearson correlation analysis and multivariate linear parameters to determine the dependency relation between stepwise regression analysis.P<0.05,indicating a statistically significant difference.[Results]l.The group of SBP,DBP,hypertension,PP,TG,LDL-C,UA,FPG levels higher than the control group,but HDL-C,Cr levels lower than the control group,with statistical difference(P<0.05);hypertension group serum 25(OH)D level(24.13±5.44ng/ml)than in the control group(27.91 ?6.03ng/ml)the level of significant differences(P<0.001).2.1n the hypertensive group,the serum level of 25(OH)D was 24.26?5.83ng/ml in male and 23.91±4.82ng/ml and 25 in female.The difference was not statistically significant(P>0.05).3.Serum 25(OH)D level was divided into 3 groups,respectively:serum 25(OH)D ? 30ng/ml is sufficient,20 ? 25(OH)D<30ng/ml is insufficient,25(OH)D<20ng/ml is deficiency;each proportion in hypertension group:22.1%,57%,20.9%;the proportion of control group in each group was 37.5%,52.3%,10.2%.The status of vitamin D deficiency(20.9%)in the hypertensive group was higher than that in the control group(10.2%),and the difference was statistically significant(P<0.05).4.Serum 25(OH)D level was correlated with SBP(r=-0.494,P<0.001),DBP(r=-0.480,P<0.001),TG(r=-0.333,P<0.001),LDL-C(r=-0.358,P<0.001),FPG(r=-0.371,P<)0.001)showed a negative correlation after Pearson correlation analysis,Serum 25(OH)D levels and HDL-C(r=-0.153,P<0.05)were positively correlated.5.Serum 25(OH)D levels with SBP(?=-0.062,P=0.020),DBP(?=-0.116,P=0.008),LDL-C(?=-2.352,P<0.001),FPG(?=-2.483,P<0.001),TG(?=-0.062,P=0.026)showed a strong correlation after multiple linear regression.[Conclusion]1.The serum vitamin D level of hypertension group is lower than that of normal persons,and the deficiency of vitamin D is more common in hypertension group.2.Analysis correlation of the level of serum 25(OH)D with the level of blood pressure,biochemical indexes,SBP,DBP,TG,LDL-C,FPG and serum 25(OH)was negatively correlated,HDL-C 25(OH)D levels were positively correlated,suggesting that 25(OH)low levels of D may be is a risk factor for hypertension.Partart ? Study on the correlation between serum 25(OH)D and vascular endothelial dysfunction in hypertension[Objective]Objective to investigate whether serum 25(OH)D is associated with vascular endothelial damage markers(EMP and NO)in hypertensive patients.[Methods]1.Research object:same as the Part ?.2.Research methods2.1 Measurement of blood pressure,height and weightThe blood pressure,height and weight of the physical examination center were measured by the physical examination center of the general examination room,and the BMI and pulse pressure difference(PP)were calculated according to formula BMI= weight/(height)2(kg/m2).2.2 Experiental materials and experimental methods2.21 Collection of speciens Anticoagulant tubes were used to extract peripheral blood 5ml in the early morning.Speciens were taken after the room temperature static,2 hours centrifugal treatment.At present,the gradient extraction method is used for the extraction of endothelial microparticles[1].Blood samples collected at room temperature were centrifuged for 10 minutes at first,then the upper serum was placed in the EP tube,the 1000g remained at room temperature for 10 minutes after centrifugation,and then the supernatant was obtained.The platelet poor plasma(PPP)was obtained by 160g.Keep PPP in-80 centigrade refrigerator for spare.2.22 Determination of indexesThe biochemical indexes such as serum total cholesterol,triglyceride,low-density lipoprotein cholesterol,high-density lipoprotein cholesterol,uric acid and creatinine were measured by automatic biochemistry instrument.2.23 Enzyme-linked immunosorbent assay was used to determine the concentration of 25(OH)D and NO in serum.2.24 The level of plasma endothelial microparticles was measured by flow cytometry.3.Statistical software analysis:Using SPSS 19 statistical software to analyze data.Continuous variables are represented by mean±standard errors.The categorical variables are expressed in terms of numbers or percentages,and logarithms of non normal distributions are transformed by logarithms.The parameters of two groups were compared with two groups of independent sample T test;count data were compared by chi square test;parameters between Pearson correlation analysis and multivariate linear parameters to determine the dependency relation between stepwise regression analysis.P<0.05,indicating a statistically significant difference.[Results]1.The level of plasma EMP was 5.60±1.41%in hypertensive group,the control group plasma EMP level was 2.56±1.42%,the plasma EMP level in hypertension group was significantly higher than the control group,the difference was statistically significant(P<0.001);serum NO level of hypertension was 45±12.28umol/L,serum NO level of control group "was53.07 ±7.38umol/L,there are significant differences between the two groups(P<0.001).2.The two groups of serum 25(OH)D levels were divided into 3 groups,respectively:serum 25(OH)D = 30ng/ml is sufficient,20 = 25(OH)D<30ng/ml,25(OH)D<20ng/ml for lack of plasma EMP levels in each group were 2.85±1.34%,4.15±1.98%6.13±1.82%,a statistically significant difference between the groups(P<0.001);serum NO levels were52.26±8.75umol/L,49.80±9.1 lumol/L,43.41±9.39umol/L,there are significant differences between the groups(P<0.001).3.Serum25(OH)D level was divided into 3 groups,respectively:serum 25(OH)D?30ng/ml is sufficient,20 ?25(OH)D<30ng/ml is insufficient,25(OH)D<20ng/ml is deficiency;plasma EMP levels in each group were 2.85±1.34%,4.15±1.98%6.13±1.82%,a statistically significant difference between the groups(P<0.001);serum NO levels were 52.26±8.75umol/L,49.80±9.1 lumol/L,43.41±9.39umol/L,there are significant differences between the groups(P<0.001).4.Multivariate linear regression EMP with blood pressure and biochemical indexes of the stepwise regression results showed that SBP(?=-0.042,P<0.001),25(OH)D(?=-0.111,P<0.001),FPG(?=0.228,P<0.001),LDL-C(?=0.310,P=0.015)showed a strong correlation.[Conclusion]1.The level of plasma EMP in patients with hypertension was significantly higher than that in the control group,while the serum NO level was significantly lower than that in the control group,indicating that patients with hypertension were more likely to suffer from vascular endothelial damage.2.Serum 25(OH)D level and vascular endothelial injury index(EMP,NO)for Pearson correlation analysis,found that EMP was negatively correlated with the level of 25(OH)D,NO was positively correlated with 25(OH)D,suggesting that the 25(OH)risk D low level may be associated with vascular endothelial damage due to hypertension.
