Objective:To conduct follow-up studies on non-diabetic people over 40 years old in Luzhou city, general features and metabolic indexes were compared between research results and the baseline, and risk factors of non-diabetic population prognosis were analyzed, which could provide diabetes or prediabetes precautions with theoretical basis and clues in Luzhou city.Method : Residents over 40 years old in Longmatan,Dashanping and Baolaiqiao communities were hierarchical cluster sampling recruited in this epidemiological study(2011-04 to 2011-11).By means of reviewing questionnaires, physical examinations and laboratory tests 3 years later,non-diabetic metabolic features,follow-up prognosis condition and its affecting risk factors were compared and analyzed.Result:1.The proportion of NGT in baseline population was 47.2%,and the proportion of IGR patients in each group was IGT>IFG/IGT>IFG,which was 22.3%?4.3%?2.4%,respectively.And diabetes accounted for 23.8% of the total population at baseline. With the increase of age,no matter in male or female,DM prevalence trend of increased gradually.Compared with NGT group, hypertensive morbidity in IFG group,IGT group,IFG/IGT group and DM group were significantly higher. In addition,DM>IFG/IGT>IGT>IFG>NGT.Obesity and the prevalence of Dyslipidemia in NGT group were significantly lower than the DM group, the IFG/IGT group,IGT and IFG group.2.The diabetes incidence accumulated in 3 years was 12.1% in reviewed non-diabetic population, with 6.9% in NGT group,12.5% in IFG group,17.0% in IGT group and 33.3% in IFG/IGT group.IFG/IGT group had the highest accumulated prognosis rate, followed by IGT group,IFG group and NGT group. The overall prognosis rate in NGT prediabetes population was 25.8%,to specific,NGT-IGT group was higher than NGT-IFG/IGTgroup and NGT-IFG group.3.There were statistical differences in Diabetes incidence between different age groups, and no matter in NGT group, the IGR group or the overall, diabetes rates are increased with age.4.Diabetes prevalence in hypertension population prevalence were higher than normal blood pressure patients,no matter in NGT group or in IGR group. In the follow-up of the crowd, the risk of diabetes in hypertension population was 5.32 times that of the normal population.Dyslipidemia population diabetes prevalence is significantly higher than normal lipid group diabetes prevalence. In the follow-up of the crowd, the risk of diabetes in Dyslipidemia people was 2.536 times that of the normal population,Prevalence of overweight or obese diabetic population increased significantly than the normal weight group.Morbidity of Diabetes in overweight or obese people was 2.279 times of normal people.5.Increased triglyceride(TG),age, creatinine(Crea),body mass index(BMI),hypertension were the risk factors in NGT-IGR prognosis from one-factor logistic regression analysis;risk factors affecting NGT-DM prognosis were elevated uric acid(UA)?hypertension, hepatic dysfunction,BMI?TG,total cholesterol(TC)and Crea.Hypertension, hepatic dysfunction, age,TC ? TG ? Crea and BMIwere the risk factors in IGR-DM prognosis.6.Increased age,UAand Crea were the risk factors in NGT-IGR prognosis from multi-factor logistic regression analysis;risk factors affecting NGT-DM prognosis were hypertension, hepatic dysfunction, age,TC and Crea.Hypertension, hepatic dysfunction, age,TG and BMI were the risk factors in IGR-DM prognosis. Conclusion:High diabetes incidence was observed in non-diabetic population in Luzhou city, among which, patients of hypertension, dislipidemia and prediabetes were the high-risk population.A good control over blood pressure and blood lipid had positive effect on glucose metabolism improvement. Objective To investigate the relationships between the levels of adipokine angiopoietin-like protein4(Angptl4)and the conversion of glucose metabolism in 3 years with normal glucose tolerance population. Methods The objects of this study were selected from the epidemiological study of cancer risk of Chinese patientswith type 2 diabetes mellitus(T2DM)in Luzhou area.A total of 276 cases with normal glucose tolerance were selected from the base-line survey, and they were divided into five groups including NGT-NGT group(normal glucose tolerance),NGT-IFG group(deteriorated into impaired fasting glucose),NGT-IGT group(deteriorated into impaired glucose tolerance),NGT-IFG/IGT group(deteriorated into IFG and IGT),NGT-DM group(deteriorated into diabetes mellitus).the level of serum Angptl4,fasting insulin(INS),and glucose and lipid metabolism parameters were detected, and the BIM,WHR and insulin resistance index(HOMA-IR)were calculated. Results Comparing to NGT-NGT group, the levels of fasting insulin and insulin resistance index in NGT-IFG group,NGT-IGT group,NGT-IFG/IGT group,NGT-DM group were elevated significantly with statistical significance(P<0.05);while Angptl4 level was remarkably reduces in NGT-NGT group, followed by NGT-IGT group,NGT-IFG group,NGT-IFG/IGT group and NGT-DM group.Negative correlations were observed between serum Angptl4 level and BMI,glycosylated hemoglobin, triglyceride,INS and insulin resistance index respectively.Multiple regression analysis indicated the independent factors for serum Angptl4 were Hb A1 c,TG and INS(? value were accordingly-0.357,-0.311 and-0.253).Conclusion Serum Angptl4 level was closely related to glucose metabolism prognosis, lipid metabolic regulation and body fat distribution in population with normal glucose tolerance.
