Based On FMRI Technology, The Brain Function Connection Network Response Characteristics Of Massage Treatment Of Simple Obesity Central Mechanism Were Studied

Objective:1.To evaluate the clinical treatment effective indexes of massage therapy for simple obesity by taking the simple obese patients as the study object,while the healthy volunteers as the baseline control.2.From the perspective of the energy metabolism balance and the appetite regulation network system of hypothalamus,with the lateral hypothalamic area(LHA)focused on,the characteristics of central mechanism underling massage therapy to treat simple obesity was studied by using functional magnetic resonance imaging(fMRI),providing some scientific evidence foe the use of massage therapy to treat simple obesity.Methods:The study adopts the way of comparison of subjects themselves before and after.A total of 30 included simple obese patients were intervened with massage therapy and a total of 10 gender and age matched healthy subjects were also included as the comparison with simple obese patients of baseline and after treatment control.All the subjects(simple obese patients and healthy subjects)were measured of weight,Body Mass Index(BMI),body fat rate(F%),waist circumference(WC),hip circumference(HC),and thigh circumference(TC),evaluated by medical outcome study short form 36 scales(SF-36 scales),the self-rating depression scale(SDS)as well as self-rating anxiety scale(SAS),and performed by fMRI scans in the baseline.After 6 weeks of massage therapy treatment,simple obese patients were measured of weight,BMI,F%,WC,HC,and TC,evaluated by SF-36 scales,and also performed by fMRI scans for the second time.Healthy subjects were scanned only once,with no intervention.Using resting state functional connectivity algorithm,LHA was selected as region of seed to calculate its resting state functional connectivity in difference between simple obese patients(before and after treanment)and healthy subjects,as well as the characteristics of the regulatory effects by massage therapy on LHA resting state functional connectivity in simple obese patients.Results1.Results of clinical dataBefore the massage therapy treatment,four aspects-PF,GH,VT and MH of SF-36 scales scores in patients of massage group was lower than normal group(P<0.05).No statistical significance existed between the depression as well as anxiety within the two group(P>0.05).The improvement of weight,BMI,F%,WC,HC and TC as well as the scores of SF-36 scales were more significantly in patients of massage group after 6 weeks treatment(P<0.05).Especially in PF,GH and VT 3 aspects(P<0.01).2.Results of LHA resting state functional connectivityCompared with health subjects,the resting state functional connectivity of LHA in simple obese patients before treatment with left inferior parietal lobule,right superior frontal gyrus,left middle frontal gyrus,left cuneus was increased while bilateral putamen/thalamus/caudate was decreased.After treatment,compared with health subjects,the resting state functional connectivity of LHA in simple obese patients with left right inferior parietal lobule,left inferior parietal lobule,right superior/middle frontal gyrus,calcarine was increased while right vermis was decreased.Compared with before treatment,the resting state functional connectivity of LHA in simple obese patients with right postcentral gyrus was increased while left superior/middle frontal gyrus was decreased after 6 weeks treatment.After regression of age,gender,disease duration,the resting state functional connectivity of LHA with lentiform nucleus positively while occipital lobe negatively associate with weight.After regression of age,gender,disease duration,the resting state functional connectivity of LHA with cerebellum anterior lobe and brainstem positively while inferior parietal lobule and temporal lobe/fusiform gyrus negatively associated with WC.Conclusions:1.There is a negative difference between simple obese patients and health subjects in quality of life and emotional status and massage therapy can improve more significantly in weight,BMI,F%,WC and HC of simple obese patients,improving the quality of life and emotional problems.2.The function of energy metabolism network system of hypothalamus in simple obese patients are abnormal,which might be one of the central pathological changes characteristic of obesity.3.There is a difference between simple obese patients and normal people in brain area of cognitive function system.4.The treatment effect of massage therapy in simple obese patients might work though somatosensory area,limbic system and reward feedback system which integrated with high cognitive function system.
