Objectives:This study was designed based on the concept of multimodal analgesia. We evaluated the effect of traditional chinese medicine analgesia combined with patient-controlled intravenous analgesia (PCIA) on analgesic effect, additional analgesic drug use, early function recovery and postoperative side reactions after THA.Explore the application value of chinese medicine analgesia combined with patient-controlled intravenous analgesia (PCIA).Method:This was a prospective,randomized controlled clinical study.60 cases undergoing THA were collected from 2014 October to 2015 December in the Affiliated Hospital of Chengdu University of Traditional Chinese Medicine.They were randomized divided into two groups. The study group patients(n=30) received traditional chinese medicine analgesia combined with patient-controlled intravenous analgesia (PCIA), and the control group patients (n=30) received mainly patient-controlled intravenous analgesia (PCIA)treatment.Postoperative pain was assessed at rest and during movement using a visual analog scale (VAS),VAS score were recorded at 4,8,12,24,48,72h postoperatively and the day checkout.The hip joint rang of motion was measured at 24h,48h,72h postoperatively and the fifth day after surgey.The side effects and additional analgesic drug use was also recorded.Postoperative 1 month follow-up,Harris score and gross satisfaction scales(GSS) were recorded.Using the statistical analysis to evaluate the efficacy and safety of study groups.Results:The mean VAS score at each time of study group was lower than of the control group,but only the rest VAS score dates at 4,8,12,24h postoperation and the movement VAS score dates at 4,8,12,24,72h postoperation were significant different(P<0.05).The hip joint rang of movement of the study group was better than the control group on postoperative day 3 and 5(P<0.05),and hip antexion ROM on postoperative day 1 was higher than control group(P<0.05).The control group used more additional analgesic drugs than the study group(P<0.05),and had a higher rate of vomit and dizzy(P<0.05).No serious complications occurred(such as infection,deep vein thrombosis)in this study.There was no significant difference between the two groups in Harris score and gross satisfaction scales(GSS) at 1 month follow-up.Conclusion:The multimodal analgesia regime combining traditional Chinese medicine analgesia and PCIA significantly improved postoperative VAS score, reduced additional analgesic drug use,increased active ROM and decreased side reactions after THA,without increasing the risks of infection and other complications.
