| Professor Zhou Dean is a master in acupuncture and the professor I’ve followed in the past three years. In this paper a comprehensive summary has been made based on my understanding of his academic theory and clinical experience.1. Professor Zhou Dean’s The Academic OriginProfessor Zhou Dean had TCM education in academic school and therefore developed a profound foundation of knowledge in Chinese medicine. Moreover, while working and studying with acupuncture masters, for example Dr. Wang Leting and Dr. He Puren in our department, his skill was fully improved. The acupuncture theory proposed by him origins from his reading, studying and rethinking of daily clinic. Based on these habits, he eventually proposed the theory of six treatments with acupuncture.2. The Introduction of Zhou’s theory2.1 the priority of treatment for "spirit"The treatment of spirit refers to acupuncture treatment for people’s spirit and mind. Visceral manifestation theory of traditional Chinese medicine indicates that the condition of viscera and bowels can reflect on the surface of the body, which is one of the most important means for diagnosing diseases in TCM. According to this, professor Zhou thought whether spirit of human is normal is relevant to diseases we have. Therefore, people should take good care of spirit on the first place. Professor Zhou then put forward the theory that emphasizes the need to treat the spirit first in traditional Chinese acupuncture.2.2 the innovative treatment for "phlegm""Phlegm" here is a unique notion from traditional Chinese medicine theory, KRM teacher emphasized the importance of thoroughly recognizing the significance of phlegm. In the process of disease forming, Chinese medicine classics have the points of view that many diseases are due to phlegm. Professor Zhou also indicated that phlegm has a huge effect on diseases since it has an important impact on the function of qi, blood and meridian inside the human body. Many diseases are accompanied by the emergence of phlegm or even caused by phlegm. Additionally, phlegm is easy to form but difficult to be cleaned. Given these characteristics of phlegm, there are acupuncture and moxibustion methods in the Zhou’s theory to tickle problems caused by phlegm.2.3 the regulation of qi for "Pain" treatmentPain is a common complication in many different kinds of diseases. Professor Zhou has a unique methodology for the diagnosis and treatment of pain diseases. He follows the location of pain and chooses acupoints from the perspective of TCM Syndrome Differentiation. In addition, for the treatment of neck, shoulder and lower back pain, he developed a set of unique methods of acupuncture treatment. Theoretically, Zhou thinks it is qi stagnation that causes pain.. Since liver is the body’s major organ for regulating Qi, liver manages all kinds of Qi. Therefore, he believes that in order to relieve pain, doctors need to start from regulating the liver function, in combine with the liver qi, to relieve the pain. Hence he created a role having adjusting qi acupuncture analgesia.2.4 the systematic treatment for "wind"The wind evil is also one of the traditional Chinese medicine pathogenic pathogens, which can cause many diseases. For the wind, the theory of traditional Chinese medicine separates it into inside and outside. Outside wind refers to the body symptom caused by external factors and skewed paralysis, while the inside wind is a series of wind syndrome caused by a variety of reasons inside the body, including facial paralysis, adverse physical activities, unclear mind, etc. Professor Zhou makes four on-off methods as the treatment for disease of wind. In his opinion, wind disease belongs to the liver’s illness. The liver’s upbearing qi may cause liver wind agitation, which shows the above series of wind syndrome.2.5 the proposal of treatment for "hyperactivity" symptomThe so-called hyperactivity symptom that first proposed by Zhou, refers to the hyperactivity disorder that usually occurs on children with various hyperactivity anxiety symptoms. In Chinese medical theory, hyperactivity disorder is considered a complicated internal disease and causes by disorders of human organs. In general, multi-hyperactivity disorder can by classified by the deficiency and sufficiency. In details, deficiency syndrome of" hyperactivity " refers to a disorder of the blood because the body is lack in internal organs, causing tendons without nourishing. Thereby the mind and other organs function was interrupted and demonstrated a variety of hyperactivity disorder. The sufficiency syndrome refers to the body hyperactivity which is commonly caused by constrained liver qi, excess of yang qi and phlegm. Generally, hyperactivity disorders can bring children a decline of focusing, attention and fidgety. For this, Professor Zhou deals with the phlegm with the method of acupuncture and drug treatment.2.6 the unique treatment to "deafness"The so-called deafness here refers to the symptoms of deafness and tinnitus caused by a variety of reasons. Patients cured by Professor Zhou clinically are almost caused by nerve damages. Professor Zhou has his unique method, which uses acupuncture in combination of herbal treatment. Moreover, in the specific treatment process, it is necessary to distinguish the symptoms, which can be acute or chronic, as well as deficiency or sufficiency. Generally, deafness is caused by the illness of liver and gall and the up going of yang qi, which should be treated by the medicine to clear liver qi and gallbladder’s excessive fire. And the long-term chronic deafness caused by the deficiency of such as qi and blood should be given priority to tonic.3. Overview of Zhou’s clinical cases The use of the theory of "six treatments with Acupuncture" covers a big range of diseases in clinic and not limited to the content outlined above of treating the three main syndromes and three main diseases. In this paper, the treatment of following diseases with the theory "six treatments with Acupuncture" will be listed:enuresis, diabetes insipidus, urinary retentron, interstitialcystitis, myastheniagravis, ataxia hereditaria, spasmodic torticollis, allergic rhinitis, myopia, diplopia, varicocele and parkinson’s disease.PurposeTo evaluate the effect of acupuncture treatment with tiao qi zhi tong formula on primary dysmenorrhea in TCM syndrome differentiation of qi stagnation caused blood stasis or cold evil caused blood stasis.DesignThis is a single blinded, randomized, controlled clinical trial. Patients who met the standard of primary dysmenorrhea were recruited in the aitio of 1:1. All patients were randomly divided into either the treatment group or the control group. In treatment group, the acupuncture treatment was applied with according to the " tiao qi zhi tong formula" (column shortage, hong, li kou)+3 vaginal intercourse, return, menstrual cramps before 3~10 days of starting treatment, acupuncture 1 time a day, the needle after needle to menstrual cramps. Continuous treatment of three menstrual cycles course of treatment. In the control group, only the emergency treatment was applied. The main outcomes included the VAS score and TCM symptom scores, duration of pain and the times of medication use. All outcomes were measured one menstrual cycle before treatment and monthly during the treatment period and the following 3 months after treatment.ResultsDuring the study,60 participants were included with 30 cases in the treatment group and 30 cases in the control group.27 cases in the treatment group and 30 cases in control group had completed the whole study. The drop-out rate was 5%.1, For the VAS score, during treatment the scores of VAS pain rating scale changes in the treatment group were greater than the control group (P< 0.001) with statistically significance. After the treatment of the first, the second and the third month, the scores of VAS were significantly different between the treatment group and control group(P< 0.001), which indicated that the effect of acupuncture treatment is better than the control group in improving the degree of pain for patients with primary dysmenorrhea. In follow-up period, at first month, the second month and the third month, the differences of the change score on VAS in treatment group and control group were also significantly different (P< 0.001), suggesting that after three months post treatment the treatment group was still superior to control group in alleviating pain.2, For the TCM symptom scores, during treatment the scores of TCM symptom changes in the treatment group were greater than the control group (P< 0.001) with statistically significance. After the treatment of the first, the second and the third month, the scores of TCM symptom were significantly different between the treatment group and control group (P < 0.001), which indicated that the effect of acupuncture treatment is better than the control group in improving symptom scores of traditional Chinese medicine. In follow-up period, at first month, the second month and the third month, the differences of the change on TCM symptom scores in treatment group and control group were also significantly different (P< 0.001), suggesting that after three months post treatment the treatment group was still superior to control group in improving TCM symptom.3, For the duration of pain, during treatment the scores of TCM symptom changes in the treatment group were greater than the control group (P< 0.001) with statistically significance. After the treatment of the first, the second and the third month, the duration of pain were significantly different between the treatment group and control group (P< 0.05), which indicated that the effect of acupuncture treatment is better than the control group in reducing the duration of pain. In follow-up period, at first month, the second month and the third month, the differences of the change on duration of pain in treatment group and control group were also significantly different (P< 0.001), suggesting that after three months post treatment the treatment group was still superior to control group in reducing the duration of pain.4, For the times of medication use, during treatment period the treatment group and control group failed to show difference with significance (P> 0.05).5, No severe adverse events occurred during this study.ConclusionThis study applied the "tiao qi zhi tong formula" of Master Zhou’s in early date than the original treatment schedule, making the acupuncture treatment as a prophylactic therapy. In this way, the clinical practice of this treatment formula is extended. The results of this study showed that acupuncture with the "tiao qi zhi tong formula" is effective on patients with primary dysmenorrhea in TCM syndrome differentiation of qi stagnation caused blood stasis or cold evil caused blood stasis. Acupuncture could decrease the pain intensity and the latency of pain attack, as well as improve the quality of life for those patients. This therapy is a simple and safe way for the treatment of primary dysmenorrhea and valuable for wide application in the clinic. |
