OBJECTIVETo investigate the operation approach to Extrapedicular percutaneous vertebroplasty In the thoracic and lumbar spine and make an anatomic study so as to offer more safe and reliable operation approaches to percutaneous vertebroplasty and avoid the dangerous complications of thoracic transpedicular operation. The results of the study were applied to the clinical operation to verify the clinical effect of the approach.METHODS 1. Our hospital radiology department underwent 64-slice spiral CT examination of 100 patients with three-dimensional reconstruction of CT the thoracolumbar data of radiological anatomy study, to measure the three approaches of data, and the measurement data were statistically analyzed.2. 32 cases of osteoporotic vertebral compression fractures were selected and randomly divided into two groups,usinginto the bilateral pedicle approach and unilateral pedicle vertebroplasty surgery, recording preoperative and postoperative VAS pain scores, kyphosis degree of improvement and operation time, fluoroscopy times, sick vertebra cement injection quantity, Analyze the data by statistics and evaluate the clinical significance of the vertebral body forming by the external approach.RESULTS 1. Compared to the thoracolumbar pedicle outside into the road vertebroplasty surgical path with the traditional approach is safe, reliable, simple and practical advantages and theoretical biomechanics superiority, posterior thoracic and lumbar vertebroplasty kinds of good choices2. Comparing the unilateral pedicle approach with bilateral pedicle approach,there was no significant difference(P> 0.05) in postoperative pain VAS score, the degree of improvement and sick vertebra cement injection quantity, there were significant differences in operative time and number of perspective(P<0.01).CONCUSION The external approach of thoracolumbar vertebral pedicle has advantages of safer, more biomechanical superiority in anatomical studies compared with approach of vertebral pedicle;in the clinical treatment of thoracolumbar osteoporotic compression fractures, both of the approaches can achieve the same good therapeutic effect,and has advantages such as shorter operative time,fewer numbers of fluoroscopy exposure and lower cost.
