Objective: On the data of operation and postoperative follow-up,the patients with anterior and posterior approach were compared with those of the patients after surgery, and the clinical effects of different operative procedures were analyzed, and then the effectiveness and safety of the combined anterior and posterior approach were analyzed, and the evidence was provided for the treatment of the lower lumbar tuberculosis with the combined anterior and posterior approach in evidence-based medicine. Methods: A retrospective analysis of the lower lumbar tuberculosis patients underwent surgical treatment at the first affiliated Hospital of Qinghai University from October 2014 to January 2017.The study collected 44 patients with lower lumbar spine tuberculosis,in 26 patients with posterior approach,18 cases were treated by anterior and post operation, including 27 cases of male patients,17 cases of female patients, simple posterior surgery Group,15 cases, female 11 cases, anterior and posterior approach combined Operation Group 12 cases, female 6 cases. The JOA scores of preoperative, postoperative and final follow-up were compared in each group, postoperative bed time, hospitalization expenses, operation time, intraoperative bleeding, postoperative bone graft fusion time, postoperative anti-TB drug use time, complication incidence, and posterior convex cobb angle change, and instruct patients in 3rd,6 after operation,9,12 months to our hospital outpatient or inpatient department for regular follow-up.Results: In the operation time, intraoperative bleeding volume,in the case of postoperative hospitalization time and hospitalization expenses, compared with anterior and posterior combined operation, simple posterior surgery had obvious advantages, and there was no significant difference between postoperative bed time, postoperative complication number, postoperative follow-up JOA score, and anterior and posterior approach combined operation in bone graft fusion time,After operation, the time of anti-TB drugs, the degree of correction of kyphosis and the correction rate were superior to the simple posterior approach. Conclusion: the anterior posterior combined operation is a safe and effective method for the treatment of the lower lumbar tuberculosis, which is a reasonable choice for the treatment of lower lumbar tuberculosis and has a certain clinical value.
