Objective The value of bile carbohydrate antigen 19-9(CA19-9)and bile protein Spermatogenesis associated 20(SSP411;also named SPATA20)alone and in combination for the diagnosis of cholangiocarcinoma was investigated.Methods From October 2016 to December 2017,85 patients were recruited for concurrent surgery.According to the postoperative pathological results,they were divided into 36 cases of bile duct group and 49 cases of biliary stone disease group.Serum samples were taken from these 85 patients before operation.The bile samples were preoperatively treated with ERCP and PTCD,intraoperative bile extraction and postoperative placement.In the tube,the levels of CA19-9 and SSP411 in serum and bile were measured,and the value of statistical analysis was used to infer the value of clinical diagnosis of cholangiocarcinoma.Results(1).The expression of CA19-9 and SSP411 in bile can distinguish the patients with cholangiocarcinoma and of biliary stone disease clearly,the diagnostic value of cholangiocarcinoma is significantly higher than that of patients with of biliary stone disease,the difference is statistically significant(p <0.05).(2).The levels of CA19-9 and SSP411 in cholangiocarcinoma group were higher than those in serum,the difference was statistically significant(p<0.05).(3).The sensitivities of CA19-9,SSP411 and combined detection in bile were 91.70%,88.90% and 94.40% respectively,and the specificity was 61.20%,79.60% and 91.80%,respectively,which were better than serum indexes,and the difference was statistically significant(all P <0.05).Conclusion(1).The expression levels of CA19-9 and SSP411 in bile and serum can effectively distinguish between patients with biliary tract cancer and of biliary stone diseases.The diagnostic value of bile CA19-9 and SSP411 for cholangiocarcinoma has obvious advantages over serum.(2).The sensitivity of combined detection of CA19-9 and SSP411 in bile were higher than that of bile CA19-9 and SSP411 alone,indicating that the combined detection of two markers can effectively improve the clinical diagnosis rate of cholangiocarcinoma.(3).Combined detection of CA19-9 and SSP411 in bile alone or in combination has a certain value in the diagnosis of cholangiocarcinoma.
