Objective:To retrospectively analyze the incidence of HDCP,mother and infant prognosis during the period from 2007 to 2016 at the Affiliated Hospital of Qinghai University,and screen out the risk factors associated with HDCP,providing a theoretical basis and reference for the prevention and treatment of HDCP in this area.Methods:A total of 12,427 maternal medical records were collected from Affiliated Hospital of Qinghai University from 2007 to 2016,including 588 HDCP mothers.Analyze the changes of HDCP detection rate and the proportion of each type of disease in 10 years,Through?~2 test to analyze HDCP maternal and non-sick maternal age,nationality,occupation,education level,current address elevation,BMI,times of prenatal examination and delivery mode.588 HDCP mothers were divided into two groups for five years.The t-test,rank sum test and trend test were used to compare the HDCP maternal morbidity,maternal complications and neonatal outcomes between the two groups.Finally,Whether or not the disease was used as a dependent variable,factors such as age and ethnicity were included as independent variables for logistic regression analysis,and high risk factors for HDCP in the region were screened out.Results:1.The average detection rate of HDCP was 4.73%between 2007 and 2016.The detection rate fluctuated from 3.33%to 6.47%,and it increased with time,P=0.000.The composition of HDCP during 10 years was 21.43%of pregnancy-induced hypertension,Mild preeclampsia 9.87%,severe preeclampsia56.29%,eclampsia 7.14%,chronic hypertension with preeclampsia 2.55%,pregnancy with chronic hypertension 2.72%.2.588 cases of HDCP mothers compared with non-affected mothers There were differences in age,ethnicity,times of prenatal examination,BMI,occupation,education level,elevation of current address,and mode of delivery.There was no difference in family history.3.Compared with the first 5 years and last 5 years the HDCP parturient,the pregnant,there were differences in gravidity,parity,age,times of prenatal examination,newborn weight and hospitalization day,but there were no differences in disease gestational week,delivery gestational week,BMI,delivery mode and bleeding volume.4.the incidence of HDCP maternal complications was 46.94%during the 10 years,and the complication rates the time is up trend,the neonatal unfavorable outcome is mainly premature birth,accounting for 22.28%.5.Logistic regression found that age,ethnic,BMI and elevation of current address are the risk factors for HDCP elevation;high education,occupation,are the protection factors.Conclusions:1.The incidence of HDCP in the affiliated hospital of Qinghai University from 2007 to 2016 was 4.73%,and showed an upward trend with time.HDCP had the highest proportion of preeclampsia in 588 cases.2.Compared with the first 5 years and last 5 years the HDCP parturient,the age of onset,the gravidity,parity,times of prenatal examination and the hospitalization day increased.3.HDCP patients with surgery-based delivery,but the cesarean section rate did not increase.4.The incidence of maternal complications is on the rise,with more and more complex complications.5.Neonatal adverse outcomes were mainly preterm,while the complications were low birth weight children,but the average birth weight of newborns increased.6.The risk of Tibetans is higher than that of Han nationality.Women at high altitudes are more susceptible to disease.7.The age,ethnic,BMI,and elevation of current address are the risk factors for HDCP;the degree of high education,occupation,and high number of prenatal examination are protective factors.
