Objective: Application FIB-4,BARD,and NFS scoring system for patients with hepatic cirrhosis of liver fibrosis grading and assessment of the degree of varicose veins,comparing the difference between the three,and explore the diagnostic value of the severity of hepatic cirrhosis.Methods:Through retrospective study,collected the clinical data of patients with cirrhosis,according to the general data and biochemical indexes of FIB-4,BARD,and NFS scoring system for liver fibrosis degree score,and analyze the relationship between the grade of liver function and the degree of varicose veins,used the ROC curve to analyze the diagnostic value of three methods.Results:1.In the child-pugh classification of liver function,the grade C of FIB-4and NFS was higher than grade A and grade B(P < 0.05),and the BARD score C was higher than grade A(P < 0.05),and the grade B of FIB-4,BARD and NFS was higher than grade A(P < 0.05).2.In the varicocele grade,the scores of FIB-4,BARD and NFS were higher in moderate,severe group than those in mild and no varices group(P <0.05).The scores of FIB-4 and NFS in mild group were higher than those in no variceal group(P <0.05).3.The AUC values of FIB-4,BARD and NFS were 0.720,0.575 and 0.717 respectively,the sensitivity was 73.2%,78.3% and 68.3% respectively,and the specificity was 65.9%,36.4% and 73.4% respectively,and the Youden index was 0.39,0.15,0.42 respectively;The AUC value of FIB-4,BARD and NFS score were combined to diagnose varicose veins was 0.741,the diagnostic sensitivity,specificity,and Youden index were 77.2%,65.9%,0.43 respectively.4.The AUC values of FIB-4,BARD and NFS score diagnosis moderate,severe variceal were 0.636,0.614 and 0.680 respectively,the sensitivity was 77.7%,87.6% and 76.6% respectively,and the specificity was 46.8%,37.6% and 54.1% respectively,and the Youden index was 0.25,0.25,0.31 respectively;The AUC value of FIB-4,BARD and NFS score were combined to diagnose varicose veins was 0.681,the diagnostic sensitivity,specificity,and Youden index were 77.0%,56.9%,0.34 respectively.Conclusions: 1.The higher the liver function child-pugh classification,the higher the score of FIB-4,BARD and NFS,among which the FIB-4 scoring system was more consistent with the severity of cirrhosis.2.The higher the degree of varicose veins in patients with cirrhosis,the higher the score of FIB-4,BARD and NFS,among which the FIB-4 and NFS scoring system were more consistent with the severity of cirrhosis.3.The AUC value of FIB-4 was superior to BARD and NFS in diagnosing varicose veins;In the diagnosis,the AUC value of NFS in moderate,severe varices is superior to FIB-4 and BARD,and the clinical value of NFS is higher;The AUC values of FIB-4,BARD and NFS are better than those used alone,and can be considered as a reliable method for noninvasive diagnosis of esophageal varices and their severity.
