Objective: To explore the relationship of the recurrence and deterioration among ADAM-10,MMP-2,RECK and SNIP by detecting the expression as well as clinical significance of ADAM-10,MMP-2 and RECK with kazal domain in sinonasal inverted papilloma(SNIP),so as to provide the new target and experimental evidence for the prognosis judgment and clinical treatment of SNIP.Methods: The 42 patients diagnosed as SNIP in 252 Hospital of PLA were collected to SNIP group;40 patientswith deviation of nasal septum and received partial inferior turbinectomy during the corresponding period were collected to control group;15 patients diagnosed as squamous cell carcinomas were collected to SCC group.Immunohistochemical analysis was adopted to research whether there was difference in expression of ADAM10,MMP-2 and RECK in the pathology tissues of patients among three groups.Relationship between the ADAM10,MMP-2,RECK expression and age, sex, clinical stage, deterioration and recurrence with the clinical pathology was analyzed. The Pearson straight line correlation was applied to check and analyze respective expression correlations of ADAM10,MMP-2 and RECK in SNIP tissue.Results:1 The variance analysis of clinical characteristics of patients in three groups. The indexes of age, gender, body mass index(BMI),course of disease, basic medical diseases and past medical history were not significantly different among three groups(P>0.05).2 The comparison of ADAM10,MMP-2 and RECK protein expressions of specimens in three groups: for ADAM10 positive rate, the SCC group was obviously higher than that in control group and SNIP group, and the difference was statistically significant(46.67%VS2.50%;46.67%VS30.95%;P<0.05);but the positive rate in SNIP was higher than that in control group, and the difference was statistically significant(30.95%VS2.50%;P < 0.05).For MMP-2 positive rate, the SCC group was obviously higher than that in SNIP group, and the difference was statistically significant(60.00%VS 35.71%;60.00% VS 5%;P<0.05);but the positive rate in SNIP was higher than that in control group, and the difference was statistically significant(35.71%VS5.00%;P<0.05).For RECK positive rate, the SCC group was obviously lower than that in control group and SNIP group, and the difference was statistically significant(11.59%VS 32.47%;1.59% VS 35.00%;P<0.05);the difference of positive rate between SNIP group and control group was not statistically significant(32.47% VS 35.00%;P<0.05).3 Corrlation of expressions of ADAM10,MMP-2,RECK,and clinical pathology index: In SNIP group, the ADAM10,MMP-2 and RECK were not statistically correlated gender, age or clinical staging(P > 0.05).The ADAM10 and MMP-2 of patients in reoccurrence group and malignant group were obviously higher than that in non-reoccurrence and non-malignant groups, and the difference was statistically significant(P<0.05).This indicated that,ADAM10 and MMP-2 could be correlated with SNIP reoccurrence and deterioration.4 Correlation of ADAM10,MMP-2 and RECK in SNIP tissue.Pearson straight line correlation test applied showed that, the expressions of ADAM10 and MMP-2 in SNIP tissue presented positive correlation(r=0.843,P<0.05);expressions of ADAM10 and RECK in SNIP tissue presented negative correlation(r=-0.798,P<0.05);expressions of MMP-2 and RECK in SNIP tissue presented negative correlation(r=-0.804,P <0.05).Conclusions:1 The expression levels of ADAM10 and MMP-2 are up-regulated,and the expression levels of RECK are down-regulated in SNIP tissue, which means that, ground substance breakage is the important cause of SNIP morbidity.2 The expression of ADAM10 and MMP-2 could be correlated with SNIP reoccurrence and deterioration.3 The expression of RECK are negatively correlated to expressions of ADAM10 and MMP-2,so it is speculated that RECK can inhibit tumor invasion and metastasis via negatively regulating expressions of ADAM10 and MMP-2.
