Objective:To compare the safety of laparoscopic D3 radical resection for right colon cancer with D3 radical resection for right colon and open radical D3 for right colon cancer during intraoperative,postoperative and perioperative complications.Methods:A retrospective case-control study was conducted to collect 40 patients undergoing Laparoscopic D3 Lymphadenectomy and Right Hemicolectomy for Right-sided Colon Cancer and 40 patients undergoing conventional open D3 radical right colon cancer from July 2013 to July 2017 in gastrointestinal surgery at the Affiliated Hospital of Qinghai University.,according to the surgical method,it was divided into endoscopic group?40 cases?and open group?40 cases?.All patients were operated on by the same surgical team.The intraoperative recording parameters,postoperative recovery,and incidence of postoperative complications were compared between the two groups.Results:The bleeding volume in the laparoscopic group was less than that in the open group,with statistical differences?59.67±18.75 ml,209.00±34.48 ml,P=0.003?.There was no statistical difference between laparoscopic surgery time and laparotomy group?122.8±30.82min,155.87±24.70min,P=0.012?.The number of lymph node dissections in the endoscopic group was not significantly different from that in the open group?11.17±1.71,10.50±1.84,P=0.663?.The recovery time of intestinal function in laparoscopic group was shorter than that in open group?20.33±3.42h,30.27±5.36h,P=0.025?.The recovery time of pain in laparoscopic group was shorter than that in open group?1.67±0.60d,3.50±0.89d,P=0.018?.The laparoscopic group had a shorter ingestion time than the laparotomy group?52.97±7.50h,74.17±15.85h,P=0.020?.The total cost of hospitalization in the laparoscopic group was not significantly different with that in the laparotomy group?55912.9±2297.58,53111.3±1893.10,P=0.023?.The incidence of perioperative complications in the endoscopic group was not significantly different with the open group?16.0%,22.0%,P=0.64?.Conclusions:Laparoscopic assisted radical resection of right colon cancer D3 is safe and feasible,laparoscopic surgery is less traumatic,rapid recovery,beautiful incision.There was no significant difference between the operation time and incidence of complications and hospitalization costs.
